Job overview

As a member of the Acute Care at Home team (ACAH), you will be an important part of the national 2 hour Urgent Community Response and roll out of virtual services, a ground breaking initiative that will pave the future of the NHS.

The ACAH service brings together acute & community nursing services, allowing for development of knowledge, skills and experience in a varied and exciting speciality. By supporting early discharges from hospital, as well as hospital avoidance from the community rapid intervention services, we are able to provide subacute care for patients in the comfort of their own homes. Benefits of this approach include the avoidance of deconditioning, ensuring that family involvement and independence is promoted, and ensuring that other service scan run alongside the virtual ward such as care agencies which in turn minimises disruption to the patients’ lives.

As the Nurse coordinator you will work alongside the Lead ACP’s & Operational Managers, clinicians, nurses and the Virtual ward Matrons, to facilitate effective use of our 2 hour UCR & Virtual Ward Beds.

The Nurse coordinator will provide triage assessment, clinical support, advice, assistance and professional expertise to all health care professionals contacting ACAH

 

The Service will be the access point for 2hr Urgent Community Response (CRIS) & Virtual Ward (VW) referrals within the UHNM Footprint and MPFT Community Services, taking referrals from the Integrated Care Coord Centre, and Acute Hospitals

Main duties of the job

To work as a key member of the Acute Care at Home Team to ensure professionals are able to make referrals through the services hub

To offer sound clinical advice that overseas entry into the service

To manage the allocation of work within the 2hr UCR team

To offer clinical oversight of patient caseloads in order to support the CRIS practitioners

To offer clinical support with diagnostics, onward referrals to follow on pathways

Support the Lone Working Policy within ACAH

To work in conjunction with the Lead Clinical Practitioner to provide high quality, clinical triage using knowledge, skills and critical thinking and professional judgement

To control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner

The role will include the collation of records and data so that utilisation can be reported effectively, and the promotion of the Virtual Ward across the acute trust to ensure effective patient flow and bed utilisation.

The Nurse coordinator will be expected to receive referrals into all aspects of our service, allocate workload and clinical visits, act as clinical contact for our service, support our clinical team with enquiries, case management and clinical oversight. The coordination of our referral access point will be pivotal to the smooth running of our ACAH service.

Essential Qualifications

Essential criteria
  • • Educated to degree level.
  • • Evidence of higher level study at Masters Level; Alternatively working knowledge and experience at an equivalent level
  • • Current HCPC Registration
  • • Current NMC Registration first level Nurse essential
  • • Physical Health Assessment Qualification or equivalent
  • • Leadership and Management Qualification
  • • Proficient in the use of Microsoft office: Word, Excel, access and Power-point
  • • Professional training delivery qualification or equivalent experience of delivering and facilitating in challenging environments.
  • • Portfolio of certified/accredited professional development activity undertaken in relevant areas
  • • Experience and/or qualifications in the specific area Organisational Development
  • • Safeguarding level 1-3 for Adults
