Advanced Clinical Practitioner

The Heart Centre Directorate are looking for a Cardiothoracic Advanced Clinical Practitioner to join our team!

A development opportunity will be considered for this position, and dependant on experience the successful candidate will either enter at Band 7 level or Band 8A.

The role will be during office hours covering the Royal Stoke site. Following an induction period, you will be predominately ward based with outreach to other specialities or areas for our patients at UHNM and will be responsible for assessing & delivering their treatment needs.

You need to be an enthusiastic, dynamic, proactive, innovative, and highly motivated nurse. You will have demonstrable knowledge of this field and recent experience of  within the Cardiothoracic speciality. You must possess excellent communication and interpersonal skills to enable you to work with the multidisciplinary team.

You will have undertaken a masters level qualification.

All applicants will be expected to meet the essential criteria and possess the relevant post registration experience as outlined in the person specification.

You will be expected, after your completing your induction period, to work autonomously, providing inpatient and outpatient care to our patients who are under cardiothoracic care.

You will be expected to be familiar with IT systems, document any care you have provided electronically.

Working with the team, you will initiate and support service development and improve patient experience based on emerging and best practice models of care responding to patient feedback and audit findings

The ACP has a trust-wide responsibility to provide clinical excellence through the four pillars of advanced clinical practice. The clinical practice pillar accounts for a minimum of 80% of their contracted hours of work and they practice with a significant higher level of responsibility and autonomy which is comparable to that of the level of medical staff providing the same level of service:

 

·         Practice as an exemplary role model throughout all four pillars of advance practice.

·         Advanced knowledge, skills and judgement for high level and complex decision making in an extended scope of practice.

·         Completing a full holistic health assessment using a range of assessment methods whilst working with patients, relatives and carers.

·         Request imaging and investigations to underpin and support clinical decisions and diagnosis.

·         Make independent diagnostic decisions based on the above and/or pathology or imaging results.

·         Make complex clinical decisions based on sound evidence base practice and clinical expertise.

·         Initiate prescribed treatments and medication, therapies, lifestyle advice and care.

·         Screen patients for early signs of disease and risk factors pertinent to their area of practice.

·         Carry out invasive and non-invasive diagnostic and therapeutic procedures where appropriate.

·         Refer to other health professionals as appropriate.

·         Discharge patients from hospital/services as required.

·         Maintain meticulous evidence of clinical specific competencies of advanced skills.

·         Exercise risk management using professional judgement.

·         Collaboratively work throughout all necessary multi-disciplinary members

Provide clear, clinical leadership across the whole clinical area and trust when necessary

Education and Qualification

  • Current registration with professional body such as NMC or HCPC
  • MSC degree in advanced clinical practice or equivalent Msc
  • Professional portfolio evidence the ongoing practice in the four pillars of advanced clinical practice
  • Registered independent non medical prescriber
  • Evidence of teaching, supervision and mentoring
  • Mentor/assessor or formal teaching qualification
  • Cardiothoracic experience minimum 3yrs Band 6 or above
  • Advanced life support or ILS support provider (dependent on area of practice)
