Job overview

An exciting opportunity has arisen for a Physiotherapist to be part of the Home First service here in North Staffs and Stoke-on-Trent.

This is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands Partnership University NHS Foundation Trust (MPFT).

The Home First Team prides themselves on the delivery of quality care to community patients, many with complex needs.

The team also work to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment.

This is a dynamic post which will be evaluated and adjusted to reflect the needs of the service, local population and feedback from users and staff. Your feedback and input will be essential to the development and shaping of the role.

The North Home First covers the whole of North Staffordshire and Stoke-on-Trent providing clinical support for patients in the community to either avoid potential hospital admissions or to support people returning home through the Discharge to Access pathway following a period of acute care.

Main duties of the job

Provide a quality driven, evidence based physiotherapy rehabilitation in the community setting, providing physiotherapy assessment, diagnosis and treatment plans for patients requiring physiotherapy.

Be competent as appropriate to the requirements of the role. Demonstrate at all times the Partnership Trusts Values and Behaviours.

Be a specialist physiotherapist and have a detailed professional and clinical knowledge. The post holder will be a specialist physiotherapist practitioner, responsible for the assessment and management of a caseload of patients requiring physiotherapy.

The post holder may work across various clinical specialities within the physiotherapy services and other specialties in the form of either a static or rotational post. 

Be responsible for a defined population of service users in a variety of settings, including the patients’ home.

Be skilled in the assessment of complex/specialist physiotherapy care/support needs and in the development, implementation and evaluation of care/support packages for patients and families. 

Have supervisory, training and leadership responsibilities where appropriate, and will have some responsibility for team performance and service developments/improvements.

Demonstrate initiative and be creative in finding solutions to problems. Work with the team leader undertaking delegated tasks in team management.

Detailed job description and main responsibilities

To be a member of the Home First Team and represent Physiotherapy as a senior clinician within the Home First Team over a 7 day per week period with early/late shift patterns. As a Clinician to prevent where possible hospital admissions and support within same day hospital discharges to maintain people in their own environment. Ultimately contributing to the delivery of reablement, rehabilitation or recovery within the D2A pathway.

Qualifications and Training

Essential criteria
  • Registered with the Health and Care Professional Council for Physiotherapy
  • Degree in Physiotherapy
  • Relevant specialist post graduate specialist post graduate qualification and/or course
Desirable criteria
  • Leadership qualification
  • Modules at Masters level
  • Evidence of further clinical study
  • Evidence of relevant post graduate courses in physiotherapy

Experience

Essential criteria
  • Relevant experience to the role including assessment and management of various conditions within the area of clinical speciality
  • Ability to manage time and caseload effectively
  • Working in a multi-disciplinary team
  • Supporting service users, families and carers in dealing with issues within the area of clinical speciality
  • Capable of managing, leading and motivating a team to a high level of competence and to delegate / priorities duties as required
  • Extensive skills and knowledge to provide assessment and treatment to patients with multiple long term and sub-acute health conditions
  • Experience of direct line management for junior staff
Desirable criteria
  • Experience of negotiating and conflict management in clinical situations
  • Have an extensive Physiotherapy background which may include working within the Acute setting, Community Hospital, Community Teams
  • Demonstrate detailed understanding of clinical competence required in the delivery of care
  • Appreciation of changes in healthcare provision nationally and locally

Skills, Knowledge and Abilities

Essential criteria
  • Good working knowledge of Physiotherapy Care Pathways and their objectives
  • Sound knowledge of the management of physiotherapy especially frail elderly patients.
  • Ability to assess, diagnose and treat patients
  • Demonstrate detailed and understanding of clinical competence required in the Discharge 2 Assess in Pathway 1 and Admission Avoidance
  • Ability to persuade and motivate patients in order for them to reach their maximum function
  • Clinical reasoning, clinical professional knowledge and expertise in multiple long term and sub-acute health conditions
  • Understanding of Information Governance, Data Protection, Safeguarding Vulnerable Adults
  • Knowledge of relevant NSF's and NICE Guidance
Desirable criteria
  • Understanding of wider NHS Agenda and its application to the local context
