Job overview
An exciting opportunity has arisen to join our team of forward thinking Advanced Clinical Practitioners within acute medicine.
The post holder will be based at West Suffolk Foundation Trust (WSFT) on the Acute Assessment Unit and Same Day Emergency Care, within Acute Medicine.
The Advanced Clinical Practitioner (ACP) will use a high degree of autonomy and complex decision making alongside demonstrating the four pillars of practice, Clinical Practice, Leadership and Management, Education and Research in providing healthcare that enables innovative solutions to enhance experience and improve outcomes within their speciality area.
The ACP is accountable for independent clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice. In addition to this the ACP is also responsible for appropriately referring patients to relevant specialities for any necessary inpatient or outpatient investigations as well as facilitating safe patient discharge.
Please note those applying for a Trainee Advanced Clinical Practitioner post will need to be willing to undertake an MSc in Advanced Clinical Practice.
Main duties of the job
· Undertakes an advanced clinical assessment in the face of uncertainty and utilises critical thinking to inform diagnosis and decision-making.
· Leads acute intervention for patients, recognises the acutely deteriorating patient and delivers resuscitation.
· Manages the assessment, diagnosis and plans future management of patients in an outpatient clinic, ambulatory or community setting, including the management of longterm conditions in the context of complexity and uncertainty.
· Manages problems in patients in special cases and other specialties
· Manages a multi-professional team, including the planning and management of effective discharge planning in complex situations.
· Manages end-of-life care and applies palliative care skills in the context of complexity and uncertainty
· Actively engages in acute medical service development, delivery and evaluation, including as a leader and role-model
· Develops, supervises and delivers alternative patient pathways including same-day emergency care
· Prioritises and selects patients appropriately according to the severity of their illness, including making decisions about escalation of care
· Integrates with other specialist services, including critical care

STAFF CAR PARKING
Staff Car Parking on the West Suffolk Hospital Site is eligible for individuals working outside of the core hours, 07:00 – 18:30.
For individuals working within the core hours, the Trust offers parking off site at the Bury St Edmunds Rugby Club, which is serviced by a regular shuttle bus to and from the Hospital. Details of the bus timetable can be found in the on-line starter pack on the West Suffolk Hospital website.
This parking restriction does not apply if an individual is required to work on a weekend or if they are required to complete “on call” duties within their role.

Education and qualifications
Essential criteria
- Registered healthcare professional.
- IT literate.
- Evidence of continuing professional development.
- Level 6 1st level degree (some universities require a minimum award). Willing to work towards a recognised advanced practice qualification at level 7 including advanced physical assessment and consultation skills and non-medical prescribing.
- Level 2 Maths and English (GCSE grade A*-C or O-level grades 1-6) or recognised functional skills assessment and qualification.
Desirable criteria
- Working towards a MSc in Advanced Practice.
- Evidence of prior learning at level 7.
- Postgraduate qualification or training relevant to speciality.
- •Reistered independent non-medical prescriber.
- Mentor/assessor and/or teaching qualification.
- Participation in research or audit projects.
- AL provider/instructor.
- Evidence of mandatory training updates.
- Management qualification / leadership training.
Experience and knowledge
Essential criteria
- Post registration experience, a considerable amount of which must be as a fully qualified healthcare professional within acute medicine.
- Evidence of management skills gained through theory and or practice.
- Experience of mentoring and developing staff.
- Broad post registration clinical experience.
