Hours: 22.5 Hours

Base:  15 hours per week at River Bourne Health Club, Chertsey. 7.5 hours per week working remotely for system wide project work with occasional requirement to travel to SASH and RSCH for meetings.

 

The successful applicant will work alongside a supportive, friendly and passionate Pelvic Health Physiotherapy team. The post holder will be expected to work closely with the wider Perinatal MDT and to help implement national perinatal service guidance, changes to pathways within the ASPH trust and to play a key role in the training of midwives and perinatal support workers. They will take a lead role in the advanced assessment, clinical diagnosis and treatment of complex patients with pelvic floor muscle dysfunction, including supporting pathways for management of complex perineal wound care and patients post Obstetric Anal Sphincter Injury. They will be responsible for managing waiting times within the services they support. In addition the post holder will spend 7.5 hours per week providing clinical and professional collaboration and leadership to the wider Surrey Heartlands integrated care system/ LMNS around the implementation of new perinatal services across the area including RSCH and SASH.

The post holder will:

·       Assist in the provision of care to patients, supported and supervised by a physiotherapist.

       Organise workload of the Perinatal Pelvic Health team and Clinical Leads, co-ordinating with other Clinical Office Administrators and Co-ordinators to ensure that an efficient service is provided.

·       Work with the teams to collate and enter data in national audit programmes.

·       Provide one to one treatment sessions with pelvic health patients in the Outpatient setting, progress exercises in a timely manner and apply clinical reasoning skills in exercise prescription

·       To work under indirect supervision to independently give pre-defined advice to women in the maternity setting including discharge from physiotherapy.

·       After a period of training to be solely responsible for the initial assessment of inpatient ante and post natal patients in line with guidelines for the following conditions:

  • ·       Obstetric anal sphincter injuries (OASI)/3rd and4th degree tears
  • ·       Diastasis rectus abdominal (DRAM)
  • ·       Incontinence (urine, faecal)      
  • ·       Antenatal/postnatal pelvic girdle pain

·       To manage a small outpatient caseload

·       Participate in continued personal development and independently identify learning needs.

·       To contribute to the supervision and training with Band 2 and 3 physiotherapy assistants

CLINICAL

Once competent, to be responsible, unsupervised, for implementing the standards and policies relevant to the Pelvic Health Physiotherapy Service and ASPH Perinatal Service. Within specific guidelines and following training, to be solely responsible and accountable for a clinical case load working within codes of practice and professional guidelines. This includes patient documentation on electronic patient record. 

 

Solely responsible: 

 

·       For the primary assessment of patients relevant to the scope of Band 4 role. 

·       Once competent; teach, supervise, and progress exercises for a specific caseload without direction. This may include (but not exclusively): 

-        Pelvic floor muscle exercises 

-        Core stability exercises 

-        Bladder health and retraining 

-        Bowel health 

-        Mobility 

-        Informative and exercise classes 

-        Motivating patients to exercise and address barriers to compliance. 

·       To run classes, virtual and face-to-face, for specific groups of patients, providing pre-set information as determined by the Pelvic Health Physiotherapy Service. 

·       To support women and birthing people in their pregnancy and postnatal journey by: 

·       Encouraging and providing advice on Health Promotion issues, including but not limited to, smoking cessation and dietary advice, within scope of practice. 

·       Encouraging healthy maintenance of wounds and recognising deviations from normal. 

·       To independently assess and treat the following types of patient on the post-natal ward in line with departmental guidelines.  

1.     Obstetric anal sphincter injuries(OASI)/3rd and4th degree tears

2.     Diastasis rectus abdominus (DRAM)

3.     Antenatal/postnatal pelvic girdle pain

4.     Incontinence issues

·       Document all care given and report any deviations from normal to the Pelvic Health Physiotherapy Team and other relevant healthcare professionals. 

·       To be responsible for performing given tasks and reporting any concerns to an appropriate member of the Pelvic Health Team through daily feedback. 

·       To be aware of and adhere to Trust and local policies and procedures. 

·       To provide programmes of activity that have a therapeutic basis following evidence-based guidelines laid down by the pelvic health physiotherapists. 

·       To be aware of safe practice and the need for onward referral ensuring working within own scope of practice. 

·       To be responsible for the safe use of physiotherapy equipment, adhering to the Trust medical device policy.  

 

PROFESSIONAL

·       To gain the required knowledge and skills in the relevant technical and clinical areas and be responsible for maintaining these skills under supervision and with support from the pelvic health physiotherapists. 

·       To attend external courses where appropriate and share learning with the wider team from any attended. 

·       To be responsible for and actively record your own personal development. 

·       To contribute to the induction and training of any new staff and physiotherapy students. 

·       To participate in clinical audit. 

·       To take part in the pelvic health team in-service training through attendance and delivered presentations. 

 

MANAGEMENT

·       To prioritise a delegated caseload and balance other patient related and professional activities in accordance with the service requirements. This will require occasional concentration in an unpredictable environment and may include responding to urgent referrals and occasionally unpredictable work patterns. 

·       To be able to manage potentially upsetting or emotional situations in an empathetic manner. 

·       To work within the Pelvic Health Physiotherapy Service and Perinatal Service to provide an efficient and effective service. 

·       To maintain and provide accurate and timely statistical information. 

·       To participate in the Trust Appraisal Scheme as an appraisee. 

·       To ensure that all equipment defects, accidents, and complaints are reported to line manager and the clinical lead immediately to ensure that appropriate action may be taken. 

·       To be responsible for cleaning and checking the conditions of relevant equipment. 

  • To undertake mandatory training as required by the Trust and in house training to attain necessary competencies.

 

ADMINISTRATIVE

  • Support administrators in Pelvic Health Physiotherapy team.
  • Be familiar with the necessary computer systems, and input accurate data as required.
  • Contribute to the organisation of the Pelvic Health Physiotherapy team and Perinatal Services.
  • Use the Trust’s information systems appropriately within Pelvic Health Physiotherapy to record all referrals ensuring data accuracy with patient details, referrer details and referral dates and highlighting any referrals which require urgent attention, and investigating any discrepancies or missing information.
  • Actively manage the scanning/uploading of referrals, assessments and other documents onto the Trust’s systems, as required, ensuring accurate data to the correct patient
  • File and archive clinical records on the Trust’s systems adding admin notes to patient records
  • Run daily reports and any other ad hoc reports from the Trust’s information systems
  • Set up and use spreadsheets
  • Demonstrate concentration required for data entry / analysis
  • Assist clinicians with queries relating to IT systems

·       Actively manage and be responsible for the email boxes for the Pelvic Health Physiotherapy team

·        Actively manage and directly investigate any complaints received, to know when to escalate to the Clinical Admin Team Leader. Chase up responses to complaints within optimum deadlines and forward to the correct party within the trust when required

·       Collect, open and date stamp post for the department, and disseminate as required

  • Undertake photocopying and other general clerical duties as required for the Pelvic Health Physiotherapy Team

 

COMMUNICATION

·       Demonstrate a courteous and helpful approach to patients and their families, ensuring patients feel reassured and supported. 

·       Take patient phone calls and seek advice from relevant pelvic health physiotherapists when appropriate, and to use reassurance skills that require tact and persuasion on occasion. 

·       To maintain accurate clinical records, ensuring compliance with CSP standards and local guidelines. 

·       Report untoward incidents, accidents, complaints, and any other relevant information to the pelvic health physiotherapists immediately. 

·       Participate fully as a team member, sharing knowledge and information and supporting colleagues to promote and cohesive team. 

·       To attend and be an active member in team meetings. 

·       To use interpreters when required to ensure effective communication occurs with patients who are hard of hearing or do not speak/understand enough English for the purposes of consent and cooperation for treatment. 

·       To work closely with the Maternity Support workers in supporting “hard to reach” patient groups

·       To use IT programs to develop and dispense relevant literature and communication materials for patients.

 

BUDGETARY AND RESOURCE MANAGEMENT

·       To order and monitor stock of disposable physiotherapy equipment and disposables for Perinatal Pelvic Health Service. 

·       To ensure that all equipment used is clean and set up correctly. If damaged or problem equipment is identified, the appropriate process is followed for repair, return or decommission.

 

RESEARCH/ AUDIT

·       In line with Trusts commitment to Clinical Governance to participate in clinical audit programme. 

·       Collection and management of service KPI data. 

 

ADDITIONAL

·       To be physically able to carry out physiotherapy interventions and therapeutic handling without direct supervision. There is a frequent requirement to exert moderate physical effort for several short periods during the day.
