We are looking for a dynamic and motivated Physical Activity Specialist with a passion and specialist interest in cardiac rehabilitation.

The cardiac rehabilitation service offers phases 1 to 3 to patients following heart surgery, PPCI, MI, angioplasty procedures and heart failure etc.
You will be assessing, teaching and supporting patients in their recovery on the telephone, face to face, and virtually as part of the hospital based cardiac rehabilitation programmes. As part of a multidisciplinary team you
will work with patients encouraging and supporting them to return to optimal health.

The role includes patient assessment, delivering individual and group education, supporting patients and delivering group and 1:1.

This post will involve cross site working.

 Welcome to the Epsom and St Helier University Hospitals NHS Trust. We offer an extensive range of services, including cancer, pathology, surgery, and gynecology to over 490,000 people in south west London and north east Surrey. We operate two busy general hospitals, Epsom Hospital and St Helier Hospital, and run services from other locations, including Sutton Hospital.   • Level 4 BACPR Phase IV Exercise Instructor professional qualification OR Degree in Sport and Exercise Science or Exercise Physiology.
  • Evidence of CPD relevant to treatment of Coronary Heart Disease
  • Evidence of up to date CPD portfolio

Experience

  • Experience working independently in cardiac rehabilitation
  • Comprehensive knowledge of CVD & the implications for exercise prescription
  • Experience of assessing, prescribing, delivering & adapting exercise to patients with cardiac conditions & those at high risk of a cardiac event
  • Experience of delivering cardiovascular lifestyle advice to patients and families
  • Experience working within a multi-disciplinary team
  • Advanced knowledge and understanding of the cardiac rehab process
  • Knowledge of outcome measures and evidence based practice associated with people with CVD
  • Experience in virtual/telephone clinics
