Ensure continuity of care following referrals from acute settings, GPs, carers, and other agencies.
  • Problem Identification and Escalation: Identify and document new problems or changes in condition, reporting findings to the Band 6 nurse as appropriate.
  • Accurate Record Keeping: Maintain accurate, comprehensive, and contemporaneous electronic records in line with Trust and NMC guidelines. Full training on the use of electronic record systems will be provided.
  • Liaison and Referrals: Liaise effectively with GPs, other services, and agencies, ensuring timely and appropriate referrals to optimise patient care.
  • Patient and Carer Support: Provide information, guidance, and emotional support to patients, families, and carers to promote positive health outcomes.
