Community Practice Development Nurse

Job overview

We are looking to recruit a Community Practice Development Nurse to support the Community Nursing teams and other services in the North East of Hampshire.

This role is to develop the training needs for the staff from all disciplines within community services including supporting students who are on placement in the community nursing, Intermediate care teams.

This role requires teaching experience and knowledge of the current changes in evidence based practice within community nursing, therefore knowledge of community nursing is desirable for this role.

This role also requires you to work closely with the Matron for Community nursing and the  wider Practice Development team within the trust.

There are fantastic  training opportunities to develop your CPD by completing in-house and University modules. You will be given full support in this role for a variety of different sources.

This role is full time covering 5 days per week, you will be expected to travel to community nursing teams and other sites as part of your role clinically and as part of the Community Practice Development role.

Main duties of the job

To provide effective practice development for the Community, older persons, Palliative and Therapies Directorate focusing on the Practice development of the Community nursing and Intermediate care team.

  • To facilitate clinical staff in community services in identifying opportunities for maintaining and improving standards of patient care.
  • To support and facilitate innovation in care provision in a structured and systematic way.
  • To offer professional guidance and support to clinical staff.
  • To develop guidelines and implement developments in practice, and ensure these are audited.
  • To co-ordinate education and training provision for Community Services.
  • To work clinically 40% of contracted hours.
