We are looking for a pro-active, highly motivated individual to join the discharge team at Frimley Park Hospital in the role of a Discharge Care Co Ordinator. You will be able to work both autonomously and as part of the discharge team. The applicant will be expected to be enthusiastic, organised with excellent communication and IT skills
Main duties of the job
An exciting opportunity has arisen for an experienced individual to apply to join the discharge team based at Frimley Park Hospital.
You will be supporting discharge planning for patients with complex health and social care needs, across any clinical specialty at Frimley Park Hospital. You will be required to work with members of the multidisciplinary team to guide patients and families through discharge pathways.
You will have excellent communication skills with a flair for problem solving and the ability to work well under pressure. You will need to have the ability to work flexibly and independently but also as part of a wide multidisciplinary team.

Detailed job description and main responsibilities
As part of an integrated Capacity and Discharge Team to work in partnership with social services and external stake holders, patients and their carers to proactively support and facilitate timely and safe discharge from hospital to home or onward care settings.
To provide a single point of contact on a named ward for patients, families, carers and associated people and co-ordinate/contribute to the safe and timely hospital discharge in partnership with other multi-disciplinary colleagues.
Maintain momentum of discharge planning throughout the entire process; supporting and working in partnership with other members of the MDT; doctors, nurses , occupational therapists , physiotherapists and other Hospital Assessment and Discharge Team members and constructively challenging where appropriate decisions with regards to discharge planning.
To screen all patients on admission and identify those who will require further social care assessment and input; complete the appropriate referral documentation and/or input directly to the relevant IT system to activate a referral or re-referral.

- NVQ Level 3 or equivalent experience
- Evidence of effective communication skills with wider MDT
- Recent experience of working in an acute hospital or social care setting
