Frailty Clinical Specialist Physiotherapist - We are looking for a Band 7 Occupational therapist to join our Community Therapy Team. This position is for a fixed term to cover the existing post holder’s leave. This role would suit an experienced Band 6 or current Band 7 Occupational Therapist.

Fixed term contract ending 13-September-2025

The post holder should have previous experience in working with patients with Neurological, Musculoskeletal / Orthopaedic, and complex elderly conditions. 

You will be expected to work alongside the Team Leader to promote an efficient, effective and productive rehab pathway from triage to discharge.   

You will need to be able to work well as part of a multidisciplinary team and be able to promote innovation and improvement within CTT.  You must be well organised and an excellent communicator and be willing to work flexibly. Time management skills and the ability to deal with Caseload, Service and staff demands are essential. 

Applicants must have HCPC registration to be considered for this post and have had experience working within the NHS in a clinical role. You will need to be able to travel independently between sites and to patient’s homes

The post holder will be working in the Community Therapy Team (CTT) based in SW Surrey covering Guildford and Waverley areas. The main base is at Milford Hospital with a satellite office at Guildford.  Work will involve providing rehabilitation to patients in their own homes, clinic environment and providing support to staff as required. The post holder will have professional responsibility for themselves and those in which they supervise. They will be expected to contribute at Band 7 level to the on-going development of clinical practice, standards, and research in addition to policies, protocols, guidelines and service development. The role will require the post holder to help lead and develop the Frailty Service within CTT.

•    To provide Specialist Occupational Therapy support to the Occupational therapists within the Community Therapy Team.
•    To work alongside other professional team leads to support the team to deliver rehabilitation services to the community of Guildford and Waverley area. 
•    To help develop specialist services within the community therapy team in line with national guidelines and evidence based practice. 
•    To carry a clinical caseload of varying conditions and to support the service manager in reviewing services and implementing service developments.

  • Diploma/ degree leading to inclusion on Health Professions Council Occupational Therapist register Evidence of relevant post registration training
  • Your professional knowledge acquired at degree level will be supplemented by specialist training, experience and short courses to Masters level or equivalent.
  • Advanced Clinical Assessment
  • Teaching/Mentorship courses

  • Understanding of current and evolving community and primary care services.
  • Member of the Royal College of Occupational Therapy.

Knowledge

  • Experience of working autonomously at an advanced level within relevant service (e.g. Community Setting) delivering effective patient focused care.
  • Experience of working autonomously assessing diagnosing and treating patients with neurological conditions.
  • Proven evidence of advanced organisational/leadership skills and autonomous practice.
  • Experience of audit, research & evidence based care.
  • Experience of delivering change management both personally and as a facilitator
  • Community Therapy/primary care experience
  • Post registration experience across a variety of clinical areas within the NHS.
  • Experience of working at a highly skilled band 6 or band 7 level with complex patients who have a wide variety of conditions, including patients who have a history of falls and diagnoses of dementia,orthopaedic and MSK conditions, in a rehabilitation setting.
  • Experience of working in a multidisciplinary team and contributing to team decisions.
  • Experience and understanding of treatment approaches and a range of therapeutic assessments.
  • Experience of use of audit and its use to improve interventions/service.
  • Experience of managing own caseload and prioritisation.
  • Experience of delegating to junior staff.
  • Experience of teaching junior staff, therapy assistants, students and other members of the team.
  • Experience of assessing clinical ability of staff and support personal development.
  • Involvement in appraisal system.
  • Demonstrate knowledge of clinical governance agenda.
