Job overview

The post holder will deliver patient care on a daily basis. They should be expert in all areas of nursing care from a patient’s admission to discharge on the Hospital at Home pathway. As senior nurse, they have overall responsibility for day-day management of their clinical caseload and ensure all care is delivered according to Trust policies and procedures associated records. They will manage and support a team of nurses and allied professionals in the delivery of evidence based, patient centred care, which is high standard. . They will assist in the management of staff appraisal, rosters, recruitment and the supervision of learners. They will provide advice and support to patients, relatives and members of the multi-disciplinary team. Utilising clinical audit, research, supervision of practice and teaching, they will monitor and improve standards of care by efficient and effective use of resources and liaise with the senior sister at all times. The will provide leadership on the Trusts values and behaviours.

Main duties of the job

  • To assist in leading the Hospital at Home team in the provision of high quality, research based care that is patient and family focused.
  • Work as part of an MDT Team to make decisions regarding onward referrals and across other areas within the Trust if required.
  • Visit patients in their own home & perform holistic care their health & care needs to enable the patient to remain at home.
  • Applies high quality, evidence based and individualised patient care in maintained at all times.
  • Be able to work independently without direct supervision after a period of training & coaching.
  • Actively participates in MDT meetings, team meetings, datix report feedback and informal in-house training.
  •  Acts as a role model & leader.
  • Maintains and promotes collaborative working relationships with other internal and external health and social services including voluntary and sectors.
  • Responsible for recognising the needs of the service, identifying risk and making rapid autonomous decisions, escalating to the Team Lead or Service manager.
  • To line manage and supervise junior members of the team including conducting one to one meeting, appraisals, performance reviews & ensuring health roster (rota system) is kept up to date for Hospital at Home.

Detailed job description and main responsibilities

Sister/Charge Nurse - Band 6

Are you looking for a new challenge managing acutely unwell adults in their own homes? If you want to work with a range of doctors and other healthcare professionals for this brand new Hospital at Home service – we’d like to hear from you.

The service will give some patients the opportunity to receive acute care and treatment in their own home. Patients will be admitted on a case-by-case basis.

We are looking for nurses who are keen to work in a hybrid role developing our IV services for our hospital at home patients as well as working clinically for the new service. Experience with managing IV therapy and lines is necessary.

Please note a valid driving license and access to a vehicle is essential for this role.

Qualifications

Essential criteria
  • NMC registered Nursing Qualification & evidence of Continued Professional Development (CPD)
Desirable criteria
  • Physical Assessment at Level 6 or 7 or a desire to Complete this as CPD.
  • Appropriate Teaching Qualification
  • First degree

Knowledge and Experience

Essential criteria
  • Understanding in differential Diagnosis / Physical assessment in order to formulate an impression as to why a patient may have deteriorated.
  • Experience at Band 5 or above
  • Knowledge of clinical governance issues such as complaints handling, clinical risk management.
Desirable criteria
  • Current knowledge and understanding in Adult community services
  • Experience working as part of a multidisciplinary team
