The Integrated Discharge Team are based in Redwood Annex in East Surrey Hospital with both community and social care teams which pays a fundamental role in joint collaboration and integrated Health and Social Care delivery.

We are looking for additional staff members for our Integrated Discharge team. Successful candidates will be expected to cover shifts between 08:00-16:00 on a 7 day rostered rota.

 You will be supporting patients, families, and Health and Social Care teams to help with the complex co-ordination and flow of patients moving between Acute and Community services.
