Job overview
The Digital Change Facilitator leverages their knowledge of primary care IT systems to help ensure the provision of high quality, relevant and cost-effective ICT and digital training for staff at primary care providers across Surrey Heartlands (GP surgeries and pharmacy, optometry and dental providers). The postholder will design, deliver and assess the content for a program of training courses, including the associated documentation. The delivery of training will be to provider staff, focusing on benefits
realisation linked to use of primary care clinical systems and associated software.
The post is suitable for Agile working.
The base for this role is set up as Woking, but the successful post holder may choose a base closest to their home address.
Main duties of the job
This is a technical role that requires applicants to be able to demonstrate experience of working within primary care and with detailed knowledge of the relevant IT systems.

Knowledge, Training and Experience
Essential criteria
- Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
- Evidence of success in efficient and effective change / transformation project and programme implementation in recent role
- Extensive, recent experience of working with primary care clinical and other IT/digital systems (e.g. EMIS/S1, Docman, Accurx)
- Project Management Experience and qualification e.g. Agile or equivalent
- Solid understanding of Change Management and improvement processes
- Knowledge of Caldicott Principles, Information Governance and Freedom of Information and the implications for electronic records
- Specialist knowledge and experience of mobilising change implementation / transformation projects in general practice

- Clear communicator with excellent training, writing, report writing skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences .
- Present, articulate and demonstrate information to a large and often high -level audience in a manner that fosters engagement and stakeholder buy -in
- Good agile project management skills with the ability to work on multiple projects at the same time.
- Needs to have a thorough understanding of and commitment to equality of opportunity and good working relationships both in terms of day-to-day working practices, but also in relation to management systems.
- Previous experience in project management and planning.
Physical Skills

- Good knowledge of Microsoft Office proficient in Excel, PowerPoint, Teams and Word.
