PURPOSE OF JOB:

 

·         To take the leading role in the advanced assessment, treatment, and management of children with diabetes within inpatients and outpatients.

·         To act as a clinical lead for the Paediatric diabetes services

·         To review and educate all children and young people admitted to hospital with a diagnosis of diabetes, with the aim to support early discharge so that they can be managed in their own home and reduce the likelihood of readmissions.

·         To develop comprehensive care plans for CYP, considering their physical, emotional, and social needs and adapt the treatment plan as necessary, including advanced clinical assessment of CYPs when an inpatient and post discharge through clinic and/or virtual visits.

·         To be highly skilled in the advanced clinical assessment and advanced management of CYP with diabetes, whilst being confident and experienced in autonomous clinical decision making.

·         To provide advanced information, education, advice, and support to the CYP, parents, ward staff and schools.

·         To actively lead in supporting and managing CYP and their families who have been identified as having a high HbA1c to reduce the risk of diabetes complications i.e., pump starts.

Main duties of the job

  • Use excellent communication skills
  • Lead on a designated clinic and independently manage a case load of CYPs.
  • Have advanced and extensive knowledge and clinical skills to manage CYP with diabetes, aetiology, and treatment.
  • Have experience of working autonomously and as a member of the nursing team/MDT
  • Be confident and competent in their role and have a willingness to develop their skills.
  • Have excellent organisational, prioritisation and problem-solving skills.
  • Have the experience and expertise of supporting and teaching ward staff, families, and other professionals such as schools.
  •  Have the ability to work flexibly and manage change in a positive and effective way.
  •  Demonstrate resilience with a fast paced and unpredictable workload.
  • Ensure that every area of the diabetes service is providing the most clinically beneficial, up to date and research-based treatment and delivery, alongside ensuring that it is the most cost effective.
  • Lead in quality improvement projects involving audits where required to improve service delivery.
  • Be willing to hold advanced responsibilities to manage junior PDSN’s to develop their skills and knowledge and become confident in their role.
  • Hold strong leadership and management skills, including the ability to oversee and coordinate healthcare teams, delegate tasks efficiently and ensure the delivery of high-quality patient care.

  • Registered Sick Children's Nurse (NMC Sub-Part 1) (Children)/RN Child
  • Relevant Diabetes Course
  • Previous experience within speciality

  • Completed/Completing Masters pathway
  • Completed Non Medical Prescribing Course
  • Completed Teaching Course

  • Previous experience of formal / informal teaching of patients & staff
