Trosolwg o'r swydd
A unique and exciting opportunity has arisen within the Offender Personality Disorder Pathway Team (OPDP), within Swansea Bay University Health Board. This is a partnership role with HM Prison & Probation Service (HMPPS).
The National OPDP Pathway is a consultation/ formulation led service and the Transitional Liaison Team seeks to support Welsh men between custodial sites and Approved Premises (AP’s) and treatment sites in England and Wales. The post holder will identify treatment needs and refer to local teams in Wales, whilst providing relational support during the difficult period of transition.
Men in the Criminal Justice System commonly present with complex mental health difficulties and trauma and the overarching aim of this role is to improve Men’s experience during the difficult period of Transition and to assist with reducing reoffending. You will maintain treatment gains, improve continuity, and increase access to services for this high risk, yet vulnerable male group.
We are looking for a compassionate individual who can work with a caseload of high-risk men as they transition from custody, to work flexibly and respond to need, able to work both autonomously and as part of a wider team.
The ability to speak Welsh is essential for this post.
Prif ddyletswyddau'r swydd
Working within the Offender Personality Disorder Pathway Team (OPDP), you will be joining a developing, highly specialist and supportive multidisciplinary team.
You will be responsible for building relationships and engaging with Prison and Community Teams, Probation staff and third sector organisations to facilitate the timely and smooth transition of high-risk men, from custody to the community.
This post is full time, on a permanent contract, and will involve travel across a wide geographical area. As this is a specialist post the OPDP team invests heavily in training staff. There will be funded opportunities to train in trauma and other psychological interventions as well as risk assessments. There will be opportunities to network nationally and participate in research to further develop the access to services in Wales.
The post holder will be expected to contribute to ongoing service development alongside fellow professionals and service users/ people on probation. They will receive ongoing support via managerial supervision and performance review from a senior member of the team.
The post holder will be required to work predominantly Monday to Friday 9am to 5pm, although flexibility around this will be considered, given the large geographical area covered.

Qualifications

- Registered Mental Health Nurse or equivalent professional qualification
- Post Registration study to degree level or evidence of working towards same or equivalent experience

- A commitment to continuous professional development and evidenced based practice.
Knowledge and Experience

- Experience of working at Band 5 or above within a forensic setting
- An understanding of the health needs of males within the Criminal Justice System
- An understanding of working collaboratively with male patients and all interested parties to develop relapse prevention and crisis intervention strategies
- The ability to work autonomously to full-fill the role of Transitional Liaison Mental Health Practitioner
- The ability to work in a psychological manner or have experience of working with Trauma
- Able to formulate risk assessments
- To demonstrate an understanding and awareness of issues related to lone working
- Ability to work in multi-disciplinary teams and with other agencies in a Professional, enthusiastic and self-motivated manner, with good organizational, inter personal and leadership skills
