Mae Bwrdd Iechyd Prifysgol Bae Abertawe yn cadw’r hawl i gau’r swydd wag hon ar ôl 24 os na dderbynnir nifer fawr o geisiadau addas. Rydym felly’n hybu ceisiadau cynnar i sicrhau ystyriaeth ar y swydd hon.

Bydd staff sy’n bresennol yn aros am adleoliad yn cael eu hystyried yn gyntaf ac felly rydym yn cadw’r hawl i dynnu’r hysbyseb hon yn ôl ar unrhyw adeg.

Sylwer bod ychwanegiad dros dro ar gyfer Bandiau 1,2 a 3 i adlewyrchu ymgorffori'r ychwanegiad i’r cyflog byw, sef £12 yr awr - £23,465 y flwyddyn. Bydd yr ychwanegiad dros dro hwn yn weithredol hyd nes y bydd y codiad cyflog blynyddol ar gyfer 2024/25 wedi’i gadarnhau.


Gellir cyflwyno ceisiadau yn y Gymraeg. Ni fydd ceisiadau a gyflwynir yn y Gymraeg yn cael eu trin yn llai ffafriol na chais a gyflwynwyd yn y Saesneg.


 

Trosolwg o'r swydd

Are you interested in working within a general hospital environment, providing specialist assessment and treatment for patients with both physical and mental health needs? Psychiatric Liaison may be the team for you!

The Mental Health Liaison Service is based in Morriston Hospital and also provides a service to Singleton Hospital, Neath & Port Talbot Hospital as well as Gorseinon Hospital.

We offer a liaison service to not only to the Emergency Department, but also specialist assessment and guidance throughout all Inpatient referred to our service from the Hospitals we provide a service to with SBUHB. We are a multi-disciplinary care team of professionals including Mental Health Nurses, Occupational Therapists, Psychiatrists, Healthcare In-reach workers in addition to our administration staff.

 

Prif ddyletswyddau'r swydd

Provide specialist mental health care assessments for Working age Adult patients and Older Peoples when required such as, Self-Harm assessment, Mental health emergencies, Complex psychosocial and behavioural assessments, Capacity assessment and Risk assessment.

Provide psychological support effectively and efficiently to patients with mental health needs who are temporarily in crisis, or who have mental health needs as a result of their physical illness or as a result of being hospitalised.

Advise non-psychiatry staff on outcome of assessment and formulate a medical care plan based on clinical intervention and risk management.

Provide time limited liaison follow up appointments when needed for those patients with more complex and enduring mental health needs.

Work positively and collaboratively as an integral part of the Specialist Adult Mental health services, Older Peoples Mental health services and effectively liaise with multi-agency colleagues, patients and their supporters.

Contribute to the development and quality of clinical practice of liaison work with this client group at local and national levels.

Provide mental health education, advice and training to clinical nursing staff, students nurses and medical students.

Work various shift patterns over a 7 day per week rota on a shift rotational basis.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

The Mental Health Liaison Service we offer a liaison service to not only the Emergency Department, but specialist assessment and guidance throughout all wards on both inpatient units. We are a multi-disciplinary core team of professionals including, Mental Health Nurses, Occupational Therapists, Psychiatrists, Healthcare In-reach staff and Administration staff.

There will also be an expectation of the successful candidates to work autonomously but also as part of a team,

We work a combination of shifts i.e. 7-3, 8-4 and 2-10 to provide a service outside of core hours. The long-term goal is to eventually become a 24-hour service.

Clinical presentations of the patients we support spans a wide spectrum from organic disorders, psychosis, self-harm, severe depression or somatoform disorder to adjustment reactions, delirium, dementia and abnormal health behaviour.

One of the most challenging and fascinating parts of the Liaison Practitioner role is that every day is completely different to the previous. It can be a fast paced and unpredictable, but never boring. We are a close, funny and supportive team with opportunities for learning and development. We undertake monthly team supervision as well as regular twice monthly teaching sessions facilitated by different members of the team.

Previous experience of working in mental health setting is required with a minimum of 2 years’ experience, however the most important requirement is that you have the commitment to support with the ongoing expansion and development of the Liaison service.

The ability to speak Welsh is desirable for this post; English and/ or Welsh speakers are equally welcome to apply.
