Would you like to work with a varied team of individuals who are dedicated to keeping patients living well at home? Neath Port Talbot and Swansea Community Resource Teams (CRTs) are looking for an individual to lead the Living Well Service, consisting of Community Medicines Management, Help me Quit, Community Memory Support and the Expert Patient Programme.  The post holder will work as an integral part of the CRTs, focusing on the delivery and management of the Living Well Service.

The CRTs work closely with both Neath Port Talbot and Swansea Local Authorities to provide an integrated, community based service across the Swansea Bay University Health Board footprint to help adults (over the age of 18) who require support to stay independent within their own homes. Key objectives are to help residents stay well and independent in their homes, preventing hospital admission and residential placements, and avoiding delays to hospital discharges. 

Main duties of the job

·   Supporting the health board and local authorities in redesigning community pathways within a range of public, independent and 3rd sector providers in order to meet the health needs of the local population, empowering Individuals to live well in their own homes.

Supporting the development, provision and co-ordination of innovative programmes to optimise the use of community health and wellbeing services across SBUHB footprint. 

Leading and managing the Living Well Services within NPT and Swansea CRTs: a portfolio of integrated multi-professional self-management, prevention and early intervention services/programmes (Community Medicines Management Team, Education Programmes for Patients, Help Me Quit (smoking Cessation service), Community Memory Support Team).

Support the development and delivery of equitable services referenced above to the Swansea and NPT localities.

Work in partnership with NPT and Swansea residents and their carers to promote resilient communities and develop high quality care and preventive service.

Responsibility for the embedding and analysis of Person Centered Outcome Measures to demonstrate the value of the Living Well Services within the CRTs. 

Responsible for budget management and the performance of the Living Well Services, as well as supporting health and social care to form, implement and deliver on the agreed integrated strategic direction for older and physically disabled people.

Qualifications

Essential criteria
  • A recognised professional qualification and registration in health care
  • Management qualification or evidence of competence
Desirable criteria
  • Educated to masters level or equivalent experience

Knowledge

Essential criteria
  • Knowledge of current issues and best practice relating to intermediate care and integrated service provision
  • Knowledge of clinical and data governance issues
  • Knowledge and understanding of performance management
  • Understanding and awareness of the importance of patient involvement
Desirable criteria
  • Knowledge and understanding of statutory and legislative requirements eg. health and safety
  • Knowledge and understanding of EPP and self-management agenda

Experience

  • Experience of managing and leading multidisciplinary multiprofessional teams

Applicant requirements

You must have appropriate UK professional registration.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
