Sylwer bod ychwanegiad dros dro ar gyfer Bandiau 1,2 a 3 i adlewyrchu ymgorffori'r ychwanegiad i’r cyflog byw, sef £12 yr awr - £23,465 y flwyddyn. Bydd yr ychwanegiad dros dro hwn yn weithredol hyd nes y bydd y codiad cyflog blynyddol ar gyfer 2024/25 wedi’i gadarnhau.


Gellir cyflwyno ceisiadau yn y Gymraeg. Ni fydd ceisiadau a gyflwynir yn y Gymraeg yn cael eu trin yn llai ffafriol na chais a gyflwynwyd yn y Saesneg.


 

Trosolwg o'r swydd

An exciting opportunity has arisen for a enthusiastic, flexible and highly motivated Occupational Therapist to join the Early Intervention Psychosis Team (EIP) based in Tonna Hospital.  This innovative team works collaboratively with young adults aged 14-25 years old who are experiencing early symptoms of Psychosis. The Occupational Therapist will aim to maximise independence whilst supporting engagement in meaningful occupations.  The role will involve developing links with community services/ projects as well as schools and local colleges. 

The successful candidate will need to be able to take on the challenges of managing a complex caseload, be committed to team working and improving the quality of life for individuals using a recovery-focused approach.  Occupational Therapy assessments and interventions will be underpinned by the Model of Human Occupation.  

Continued support and supervision will be provided to develop the practitioner to the band 6 role. This will be an excellent development opportunity for a band 5 looking for career development.

Prif ddyletswyddau'r swydd

This post will offer an opportunity to be based in an MDT with a strong therapeutic focus.  The OT role will be guided by the Model of Human Occupation and will allow the post holder to make informed decisions with patients, and develop shared goals and plans.

To manage a defined caseload within specialist area, using evidence based /client centred principles to assess, plan, implement and evaluate the outcomes of intervention where appropriate; hospital, community, patient's homes, schools, outpatient settings and maintaining associated records.

To organise this efficiently and effectively with regard to clinical priorities and use of time.

 To ensure clinical records are kept to agreed standards and report accurately to others. 

To work in partnership with clients, carers and all stake holders including the multidisciplinary/ multi agency team to identify relevant goals.

To provide leadership and supervision for junior staff, as delegated by supervisor /team leader.

To regularly supervise Occupational Therapy students on practice placements.

To participate in CPD and develop skills and knowledge through participation in the Band 6 Development Programme and other developmental activities.

  • Professional Registration with HCPC

Experience & Knowledge

  • Experience of autonomous caseload management, comprising assessment, planning intervention and evaluation within relevant clinical area
  • Understanding of the Occupational Therapy process with particular emphasis on physical conditions
  • Knowledge of the principles of Clinical Governance
  • Broad knowledge of current best practice in Occupational Therapy with designated client group
  • Experience of research and/ or audit and implementing evidence based practice
  • Previous experience of supervising staff
