Job overview

An exciting opportunity as arisen within the Crisis Team [Neath Port Talbot] for a Registered Mental Health Nurse. The successful candidate will work within a multidisciplinary team providing care to those individuals experiencing a Mental Health Crisis.

Ideally this should be provided in the least restrictive environment i.e. the patients home / communities. The successful candidate will also be required to participate in the Mental Health Telephone Triage.

Full time and Part time hours available

Main duties of the job

The team aims to provide high quality care within the least restrictive environment for individuals and their families. The team will encourage development and support.  

Conducting holistic assessments for individuals experiencing a mental health crisis.

Risk assessing/risk management

Telephone triage of referrals

Home treatment, providing 1:1 input, psychosocial interventions.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

To assess individuals experiencing a mental health crisis.

Facilitate early discharge from Acute mental health wards.

Risk assess, manage and formulate risk management plans.

Liaise and work within the MDT and partner agencies.
