Sylwer bod ychwanegiad dros dro ar gyfer Bandiau 1,2 a 3 i adlewyrchu ymgorffori'r ychwanegiad i’r cyflog byw, sef £12 yr awr - £23,465 y flwyddyn. Bydd yr ychwanegiad dros dro hwn yn weithredol hyd nes y bydd y codiad cyflog blynyddol ar gyfer 2024/25 wedi’i gadarnhau.

Trosolwg o'r swydd

Primary and Community Nursing services are currently looking for a motivated active NMC registered nurse/ midwife/ health visitor with a passion for infection prevention and quality improvement, to deliver safe effective care to our community patients.  The Practice Educator role will support the Service Group's Infection Prevention and Control lead to deliver education, promote best practice and value based healthcare that aligns to the Health Boards  core priories to reduce Health Care Acquired infections in our Region.  Primary Therapy and Community Services work closely with the Health Boards dynamic Infection, Prevention and Control team and other service group leads to deliver a whole system approach as well as linking participating in National Forums and linking with Public Health Wales Colleagues. 

Infection Prevention & Control (IPC) is an essential part of everyday practice in all health care settings and the responsibility of everyone.  This is a new role which will focus on quality improvement  projects, including delivering best practice education and training  to support community teams across Swansea and Neath Board Talbot. If you have a passion in improving outcomes for patients, enhance learning in Infection prevention and share that learning while improving the practices of others, we welcome your application.

The successful post holder will become an integral member of the Primary Care and Community Team as a Practice Educator in this specialist area and gain valuable experience working alongside members of the team to deliver the Service Groups Infection Prevention and Control improvement plan.  This will include undertaking site visits across the region for assurance purposes, participating in the delivery of education, training and audit programmes, supporting learning related to healthcare associated  infections via key priority projects delivered across the service group. 

You will gain valuable experience working collaboratively with many staff disciplines and Service Groups across Swansea Bay. Influencing and supporting best practice through positive role modelling and learning, providing assurance that a equitable and high standard of care is being delivered across a variety of community settings. 

The ability to speak Welsh is desirable for this post, but not essential.  English and/or Welsh speakers are equally welcome to apply.

Qualifications

  • Registered Nurse/ Midwife/ Health Visitor
  • Active NMC Registration and evidence of maintaining a portfolio to meet revalidation requirements
  • First Degree (or working towards)
  • Nationally recognised specialist qualification in IPC, or equivalent (or willingness to undertake such training within 1 year)

Experience

  • Experience in facilitating change and evidence of innovations

  • Previous experience of working in an acute care setting
  • Previous experience of working in the community/ mental health/ learning disabilities setting
  • Knowledge of alert organism surveillance and other surveillance programmes; e.g. as specified by the Welsh Government
  • Theoretical knowledge of National Infection Prevention & Control agenda

  • Good organisational skills
  • Confident, approachable and able to work effectively with a wide range of staff and users of the service
  • Ability to work under pressure and cope with change/ adjust plans quickly and responsively in the light of new information or changed circumstances
  • Excellent interpersonal skills – including observation, listening and empathy skills
  • Maintain confidentiality
  • Knowledge of standard of record keeping and ability to audit this
  • Demonstrates good analytical and reflection skills
  • Able to make clinical decisions following assessment of complex cases; where there are several/ many components to be considered and/ or where opinion differs
  • Able to identify areas for own Personal Development Plan/ Professional Portfolio; including specific objectives related to own work
