Job overview

The post holder will be responsible for the provision of reliable, consistent, and timely information on the mental health bed state, identifying capacity and demand situations and working to support the senior members of mental health services to evaluate and manage clinical risk related to length of stay, delayed transfers of care within the inpatient settings and acute out of area admissions. To support the resolution of these issues and assist to manage the patient flow and prevent  acute out of area admissions.

 

The post holder will take delegated responsibility for the escalation of specific bed management issues to the relevant clinical unit or department. Working alongside the multidisciplinary teams for each area the post holder will work innovatively and creatively to find solutions to individual issues, including identifying complex discharge issues that need to be escalated to the respective clinical or management teams.

 

The post holder will be flexible and work across all CWPT inpatient wards and will also work to  oversee acute out of area admissions, attending MDT’s, ward rounds, and supporting the discharge pathway for patients placed in private hospitals.

 

The role will require the post holder to ensure that the wards are reporting patient flow and detailed discharge plans through the mental health inpatient bed management and delayed transfer of care governance reporting system and that patients have a discharge date at the point of admission.

Main duties of the job

To monitor and manage the oversight and reporting of the standard admission to discharge process on all the mental health inpatient wards and acute out of area admissions.

 

To monitor and manage the oversight and reporting of patient length of stay in line with the development of pathway interventions in Mental Health Services.

  

To ensure they are aware of all patients who are admitted in mental health beds within 24 hours (one working day) and that an identified date of discharge is recorded within 24 hours of admission. 

 

Ensure escalation of complex delays is reported and recorded within the agreed admission to discharge framework.

 

Support Ward Managers to enable staff to be fully competent and timely in completion of CHC, JHAS paperwork.

 

 

To ensure that all patients deemed fit for discharge are reported accurately, and managed and escalated within the agreed identified processes and procedures. This will include assuring reports are undertaken in a timely manner and provided to the identified panel, professional body, and scrutiny panels.

 

The post holder will also be required to support clinical staff in presenting reports at funding panels.

 

 

Support Ward Managers to ensure timely progress of discharge planning.

 

Liaise and support with Named Nurse, Care Co-ordinator in community teams to ensure there is clear agreement on what tasks each party is required to undertake and ensuring that this is clearly documented on internal systems.

Detailed job description and main responsibilities

Following internal promotions and progression, we are delighted to offer an exciting opportunity for experienced enthusiastic clinician to join the patient flow team . The patient flow team is a small and dynamic team.   Working directly with inpatient teams and  community teams to support discharge from  mental health inpatient services.  Working collaboratively  with system partners  to ensure the safe discharge of patients  to meet their current needs. 

The Role requires : 

To have oversight of cases that are not currently allocated to the Care Co-ordinator and to assure that discharge planning is progressed until allocation happens.

 

To support the optimisation of bed utilisation and reduce the length of stay and delayed transfers of care within mental health, ensuring the timely and safe transfer of care is undertaken.

 

To manage the necessary information via internal systems and ensure that all data is live and accurately recorded.

 

Liaison with community Team Managers within the CMHT to ensure that referrals are prioritised and allocated for discharge planning purposes at the point of admission within the first 5 days from admission.

 

To ensure that the Care Co-ordinators are in attendance for necessary discharge planning starting from the point of admission. Where this is not happening the post holder will escalate and progress allocation requirements to the identified community  lead.

 

Provide regular updates to the S&Q lead for length of stay and delayed transfers of care. To provide the operation leads with the necessary weekly reports to support the DTOC LOS and Bronze escalation meetings.

 

To assist the team to update and report to the identified contracting team and support the reporting mechanisms.

 

To highlight/escalate to the Pathway/Service Manager any issues in relation to delays.

Qualifications

Essential criteria
  • Current professional registration: Nurse, Social Worker, Occupational Therapist
  • Educated to degree level or equivalent

Knowledge and skills

Essential criteria
  • Evidence of having developed expertise in a particular clinical area e.g. dementia, LD etc.
  • Ability to liaise effectively with statutory and voluntary services as well as provider organisations / placements
  • Knowledge of other services provided in the locality

Experience

Essential criteria
  • Experience of working independently as an autonomous practitioner
  • Experience of joint working with statutory, voluntary and private sector agencies
  • Experience of holding and managing a case load
