Cancer Multi-Disciplinary Team Facilitator

An exciting opportunity has arisen to join the Cancer Services team at University Hospital Coventry and Warwickshire as a MDT Facilitator. The MDT Facilitator is an essential part of the whole team providing high quality and prompt care to cancer patients.

The individual is responsible for the smooth running and efficiency of the MDT meetings; including preparation for meetings, tracking and chasing up on progress of cancer patients' diagnostic and treatment steps, timely and accurate collection of cancer data, and ensuring outcomes are followed up after MDT meetings.

We are looking for an individual to join our team who is organised, has attention to detail, good inter-personal and communication skills, and is enthusiastic and proactive in their approach to work.

This role is crucial in the tracking and management of all patients who are on suspected cancer pathways within the relevant disease site. The post-holder must be able to work independently and follow the policies and procedures for the department. The post-holder will be expected to use their own initiative and keep their line manager and senior colleagues informed.

This is a role with lots of variety and ad hoc demands and the post-holder must possess excellent time management and planning skills.

Prif ddyletswyddau'r swydd

 To work closely with clinicians, specialist nurses, secretaries and all members of the Multi-Disciplinary Team (MDT) across all specialties to ensure that suspected and diagnosed cancer patients are actively managed through patient pathways.

 To be responsible for the management of the patient’s pathway, from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process.

 To provide effective administrative support to ensure smooth facilitation and co-ordination of multi-disciplinary team meetings (MDTs) on behalf of MDT Lead, including organising agendas and typing of live outcomes within the MDT meeting.

 To be responsible for ensuring the accurate and complete collection and management of the national cancer minimum dataset for all patients discussed at MDT, and to ensure all patient pathways are correctly mapped and appropriately tracked. 

To maintain up to date knowledge of all targets and processes relating to the patient pathway process for suspected and diagnosed cancer patients and to be a point of reference within the department of other members of the MDT. 

To assist the Cancer Support Facilitators in the development and refinement of all MDT and cancer Quality Surveillance processes.

The post holder will be joining our Performance & Informatics (P&I) department who are well respected and the largest of our corporate services.  The P&I portfolio encompasses Business Intelligence & Informatics, Corporate Performance, Information Systems Development, Clinical Coding, Data Quality, Patient Access, Cancer Services and Analytical Development. 

P&I work cross functionally to support the Trusts operational delivery of both national and internal targets which ensures the highest standards of patient care are delivered.  A robust Performance Management Framework is embedded across UHCW that tracks and evaluates progress and supports both operational and clinical governance frameworks.  

New opportunities at a national level are being developed by P&I in areas of health inequality, waiting list management and system working.  Most notably working innovatively to develop the ‘Health Equity and the Right to Treatment’ (HEARTT) tool which culminated in being awarded the ‘Innovation and Improvement in Reducing Healthcare Inequalities Award 2022’, and also being recognised with the ‘Best Healthcare Analytics Award 2024’ for the collaborative work alongside IBM and Celonis regarding Outpatient Optimisation and Patient Engagement at the prestigious Health Service Journal Awards.
