Integrated Discharge OT Team Leader

Job overview

An exciting opportunity has arisen within the Integrated Discharge Team (IDT) for 1.0 WTE Band 7 OT based at UHCW site.

We are seeking an enthusiastic and passionate OT, who is keen to develop their leadership and management skills, in this specialist area. 

The successful candidate will assist in supporting the team to ensure effective, safe and timely discharges, maximising flow through the organisation, effective leadership is essential in co-coordinating the day-to-day running of the service and supporting the team through ongoing changes in service delivery. 

The candidate will work alongside senior management both within the organisation and with the wider system to further enhance, evaluate and embed a high quality integrated discharge model. 

You will provide expert knowledge in discharge processes, assessing and coordinating the discharge of complex patients as an autonomous practitioner within the team.

You will be accountable for training, supervising and educating members of the team and the wider MDT.

Main duties of the job

• Interpret and analyse clinical and non clinical facts to form accurate diagnosis and prognosis in a wide range of highly complex conditions to assess and the most appropriate discharge pathways and ensure safe timely discharge.

• Undertake comprehensive assessment of patients including those with complex presentation, investigate and analyse skills and formulate individualised management and discharge plans.

• Participate in working parties developing changes within the Integrated Discharge Team and other specialities within the group.

• Assist with the implantation of change and evaluation of changes to service provision for the service.

• Develop excellent relationships with all members of them and other appropriate therapy leads to ensure optimal delivery of services.

• Review and recommend developments for future delivery of therapy services within the discharge team ensuring the most clinically effective use of resources.

• Demonstrate effective prioritisation between clinical commitments and operational responsibilities.

Detailed job description and main responsibilities

• To assess, plan, coordinate, deliver, evaluate and manage a high quality discharge service. To ensure the service is efficient and evidence based. To provide a source of highly expert specialised knowledge in discharge. 

• Provide a highly specialist, high standard Occupational Therapy service assessing and coordinating patients care and discharge plan as an autonomous practitioner within the discharge team. 
 
• Lead the therapy staff across the service area providing clinical supervision and training, liaising with other appropriate therapy leads to ensure a seamless service is provided throughout the discharge process.

• Provide support and advice for the continuous development, co-ordination and evaluation of the appropriate pathways within discharge.  

• Provide a source of expert clinical opinion on discharge for other health professionals, both internal and external to the Trust.

• Provide knowledge and expertise on complex discharges.
