Job overview

An exciting opportunity has arisen for a Physiotherapist to join our Community band 5 rotation. The post will include rotating into our new and transformational One Coventry Integrated Team (OCIT), Community  Integrated Neuro and Stroke service (CINSS) and Planned Community Therapy. This role will enable you to develop knowledge and skills that support the future of community based rehabilitation. 

Please note that a Full UK driving license is an essential for this role.

Main duties of the job

·       To undertake the assessment and treatment of clients referred to the Community Services pathway, providing a comprehensive range of physiotherapy services. 

·       To work within an inter-disciplinary team, providing physiotherapy, planning and delivering programmes of rehabilitation.

·       To provide advice and guidance to others, clients, carers and staff working in health and social care.

·       To participate in the Physiotherapy Community Services pathway rotational scheme.

Detailed job description and main responsibilities

You will play a key role in admission prevention, hospital discharge, proactive and preventative care through rehabilitation within the community. 

You will assess patients on their own homes, in temporary or permanent residential or nursing placements to maximise independence and empower rehabilitation potential.
