This is an exciting opportunity for an experienced registered nurse to join the Anticoagulation team.
Main duties of the job
Working as part of a multi-professional team to deliver enhanced clinical practice to a specific client group and participate in education, audit and application of evidence-based practice within the specialty.
- Manage daily anticoagulation nurse led clinics
- Receive direct referrals from General Practitioners and Consultants
- Manage and co-ordinate all investigations and treatment for DVT patients from referral to diagnosis according to the DVT protocol
- Participate in approved Patient Group directions by the Trust appropriate to our sphere of practice, to maintain safe anticoagulation for patients.
- Co-ordinate patient discharge liaising with all members of the multidisciplinary team to ensure that safe delivery of anticoagulation continues as an outpatient

Detailed job description and main responsibilities
Clinical Leadership and Practice
- Carry out programmes of care and treatment determined by others
- Manages clinical caseload on a day-to-day basis including making minor modifications within prescribed parameters reporting back to the Clinical Nurse Specialist.
- Utilises advanced skills of assessment , interprets complex psychological, emotional, spiritual and cultural needs.
- Provides advice in relation to the care of individual or groups of patients.
- Liaises with other nursing staff caring for clients within the specialty in order to provide enhanced knowledge to optimise patient care.
- Demonstrates a commitment to best practice and relevant knowledge of the NHS and modernisation agenda, seeking to develop the service as part of the professional team
Audit and evidence based practice:
- Participates in research and audit as part of the multi-professional team within the specialism.
- Ensure that high quality data is collected for both audit and research trials, as required.
- In support of the Clinical Nurse Specialist, develops protocols of care and implements evidence based practice.
Communication, Relationship skills and Teamwork
- Communicates effectively with colleagues, patients and other users of the Trust.
- Produces written communications which are clear, fluent, concise and easily understood.
- Establish and build relationship with colleagues and clients/users.
- Fully understand the rights and views of clients/users, their families, seeking out and addressing their needs.
- Cooperates and works well with others appreciating the roles of other staff and agencies involved in the care of patients.
Specialist Practice

- Two years at band 5 or equivalent
- Experience within the speciality
- Experience of continuing professional development

- RGN/RSCN/RM
- Teaching Qualification
- Willing to work towards post grad qualification in speciality
