We are currently receiving significant investment aiming to transform community mental health services across the Black Country.

We are developing an Integrated Community model, with the focus on new ways of working within integrated mental health teams to support people living in their communities with mental health challenges.

 

 

 

 

Main duties of the job

You will be a Registered Mental Health Nurse, preferably with adult and community mental health experience of working with severe and enduring mental health service users working. You will be responsible for Care coordinating the service users with complex long term mental health problems who find it difficult to engage. You will have experience in assessing and formulating individual intervention plans and outcome measures for adults with complex mental health problems and have current knowledge of NHS policies and practices in mental health. You will participate and support the CMHT duty system and be responsible for managing the mental health needs of service users referred to the team.

The successful applicants will also have previous experience of the Recovery Model, have a good working knowledge of psychotropic medication and effectiveness plus the ability to deliver a range of therapeutic interventions. In addition, the post holder will demonstrate the ability to undertake and manage clinical risks and develop robust clinical management plans for Service Users within the service. You will support the team manager and clinical lead to adhere to and deliver high quality service and have in depth knowledge of national quality indicators.

The team is committed to providing safe, sound and supportive services. You will receive regular supervision, with opportunities to develop clinical and professional practice in line with Service User needs and organizational values.
