We are looking for an enthusiastic RMN nurse to take on the role of Staff nurse at our Dementia Assessment Unit. The DAU is a mixed sex ward with a Gold accreditation for a dementia enabling environment. It is part of the older peoples dementia care pathway. The unit assesses  patient needs and creates a treatment plan to enable patients to move on to the appropriate community setting to support their care needs closer to home.

The post holder will be responsible for helping the co-ordination and administering assessments of patient care needs, the development of treatment programmes and the implementation and evaluation of these  within a multidisciplinary team. The post holder will support the development of the team to ensure in depth holistic assessments are completed to include assessment relating to mental health, physical health and social needs. The post holder will also be flexible to support the Older Peoples Dementia care pathway in supporting preventable admissions to hospital and facilitating timely discharges.

This is a great opportunity for a motivated and energised person who will have the opportunity to support the continued work to ensure that  DAU provides a high level of excellent care and support. 

 

Prif ddyletswyddau'r swydd

To provide and coordinate a high standard of care to the patients cared for on the Dementia Assessment Unit.

Act as a lead practitioner to ensure that the care of patients is of the highest quality.

Supporting discharge planning from admission.

Delegate work to staff and be the responsible clinician that ensures care 
requirements are implemented satisfactorily.

Distribute medication in line with Trust policy and NMC guidelines.

Maintain administration information systems as required. 

To comply with the correct implementation of the Mental Health Act.

  • Clinical experience including experience of risk assessment, assessing patients and creating/implementing and evaluating care plans etc.
  • Experience of working within a multicultural framework
  • Experience of working with Dementia clients and their families
  • Experience of working with people with complex care needs associated with their Dementia.
