CYP Clinical Lead - Diabetes

The appointee will be a key member of a core team of 2-3 medical and non-medical professionals co-leading the delivery of regional transformation programme for  CYA (Children and Young Adults) with diabetes. The post holder will work with colleagues to provide clinical leadership, engagement and influence in paediatric and young adult diabetes working alongside other members of the Regional CYP and adult transformation teams. The role will support many aspects of care being delivered by the East of England regional paediatric and adult diabetes networks, their members and other stakeholders. A strong track record in multidisciplinary working is important from working across diabetes specialist teams as well as experience in working with young people with diabetes and their carers.

·       Work with the regional CYP and Adult Clinical Networks to support improvement in CYA Diabetes services across the East of England.

·       Provide clinical advice and leadership to the CYP Network and Adult Diabetes Network

·       Support systems and ICS Diabetes leads to roll out national and regional priorities for CYA with diabetes including transition and reducing health care inequalities.

·       Provide expert clinical advice and support to the CYP Clinical Director, CYP and Adult Diabetes Clinical Lead colleagues and Head of East of England CYP Clinical Network.

  • To engage and communicate with a wide variety of other stakeholders to promote sustainable and equitable CYA Diabetes services of the highest quality possible.
  • To act as a conduit for information and initiatives between regional and local systems.

Colleagues with a contractual office base are expected to spend, on average, at least 40% of their time working in-person.

·       To provide expert clinical support and leadership to the regional Head of CYP Clinical Network & Transformation and the wider CYA regional team.

·       Support the ICS to develop a strategic approach to integrating services for CYA, improving the quality of care for CYP with diabetes. Include CYA with diabetes in local, regional and national policy and programme design to ensure that services are designed to meet the needs of CYA and improvements are made in reducing inequalities.

·       Support the delivery of the optimal configuration of CYA services within a given geography, including collaborative development of associated capital bids and cases for change.

·       Ensure full engagement with the National CYA with diabetes Transformation Programme, monitoring network performance and recommending support.

·       Ensure effective Patient flows and care pathways across the CYA clinical network, with clinical collaboration and coordination between all stakeholders including the third sector supporting the monitoring of consistent, high quality care through the delivery of National clinical standards for CYA with diabetes.

·       Ensure collaborative working with ICSs and provider workforce leads to manage system capacity and demand.

·       Horizon scanning- participating in National forums to ensure that as healthcare technologies advance and new intelligence is introduced, local workforce are supported to develop and deliver innovations in patient care.

Qualifications

  • Registered with a relevant professional body. Educated to masters’ level or equivalent level of experience of working at a senior level in specialist area.
  • Working in paediatric and/or adult diabetes with significant experience of young adult services for a minimum of 5 years
  • Experienced health professional with credibility and the ability to command the respect of all clinical and non-clinical professionals.
  • Excellent leadership skills and the ability to motivate
  • Demonstrably involves patients, carers and the public in their work. Consistently puts children and young people at the heart of decision making. Values diversity and difference, operates with integrity and openness

Knowledge and Experience

  • Working in paediatric and/or adult diabetes with significant experience of young adult services for a minimum of 5 years
  • Knowledge and experience of working in formal clinical networks within the NHS
  • Good understanding of integrated models of care across primary, secondary, tertiary and community care.
  • Knowledge of the NDPA and associated data reports, and knowledge of Peer review reports of individual paediatric diabetes services.
  • Track record of delivering change in clinical services
  • Health professional with an identified clinical leadership experience
  • Experienced health professional with credibility and the ability to command the respect of all clinical and non-clinical professionals
  • Experience of developing, applying, and reviewing an evidence-based approach to decision making
  • Trained Diabetes Peer Reviewer
  • Active member of the East of England CYA Diabetes Regional Network or East of England adult diabetes network
