Job overview

The post holder will have a specific focus on improving all aspects of discharge supporting the wards /departments  through strong clinical leadership; service improvement and development; and improve the patient’s experience of safe, timely discharge.

The post holder is directly responsible for the line management of the Discharge Co-ordinators and the provision of leadership and direction to these staff.

Main duties of the job

  1. Part of a multi-disciplinary team within the trust Discharge Team  provide discharge planning support where necessary and case management for the most complex group of patients
  2. Ensure  patients and their families/carers are consulted and encouraged to be part of the discharge planning process
  3. Oversee the case management and co-ordination of the discharge plans for patients requiring complex support.
  4. Ensure patients on a complex discharge plan are reviewed against their actual plan and take appropriate action when there is any delay.
  5. Ensure smooth and timely communication between the Trust and partner agencies
  6. To support the Trust in managing the patient pathway and flow
  7. Ensure that all areas within the Trust are aware of key issues pertaining to discharge planning.
  8. Act as a role model for all staff in the organisation
  9. To ensure the key performance indicators relating to discharge planning are met.

Detailed job description and main responsibilities

  6. To support the Trust in managing the patient pathway and flow including recognition of the need for escalation of delays in transfers of care
  7. Ensure that all areas within the Trust are aware of key issues pertaining to discharge planning.
  8. Act as a role model for all staff in the organisation and demonstrate the standards, values and behaviours as expected.
  9. To ensure the key performance indicators relating to discharge planning are met.

Person specification

Qualifications

Essential criteria
  • Relevant Degree in own clinical speciality
  • Evidence of Continuing Professional Development in specialist field of practice

Experience

Essential criteria
  • Experience of utilising evidence-based practice
  • Recent and relevant operational management experience and demonstrable clinical experience/expertise.
Desirable criteria
  • Involvement with clinical networks/patient pathways within the Trust.
  • Evidence of working at a strategic level
  • Project work within own organisation. Involvement with a modernisation /improvement project.

Special Knowledge/Expertise

Essential criteria
  • Competent in development and delivery of programmes of care.
  • Ability to support the performance against Trust targets.
  • Understanding the principles and processes of Service Change
  • An in depth understanding and experience of patient flow/strategic bed management/discharge pathways across the health economy.
  • Experience in staff or student supervision
  • Experience in managing a ward or clinical department OR experience in working with inpatients and the community setting

Training

Essential criteria
  • The job holder must take responsibility with their line a manager for personal development and ensuring continuous professional development is a priority
  • Will undertake all mandatory training for the role
  • Identify and support delivery of training requirements for own area
