Are you a qualified nursing associate looking for an exciting new challenge and would like to be part of our new Acute response Team if so
We are searching for Band 4 Nursing Associates to come and join our new and evolving nursing team in CHFT Acute Response service.
This service will support the deteriorating patient on both sites and will be working with the new integrated team (formerly Critical Care Outreach and Hospital Out of Hours).
The post holder will work as an integral member of the Acute response team, providing an exemplary service to deteriorating patients.
The post holder will utilise an appropriate assessment tool to determine allocation of tasks to skilled professionals, as well as utilise own learned skills, to include: triaging patients, venepuncture and cannulation, collection of blood cultures, ultrasound guided cannulation, venous blood gas sampling, spirometry out of hours,
Be involved in informal and formal education to other healthcare professionals within the Trust.
This role will cover out of hours, predominantly nights, weekend days and nights, and bank holidays.
The post holder will be required to rotate between both sites that make up Calderdale & Huddersfield NHS Foundation Trust.

Working for our organisation
We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients’ homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.
We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children’s and young people’s services; end of life care and outpatient and diagnostic imaging services.
We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.

Detailed job description and main responsibilities
Work as part of a designated clinical and care team delivering care that focuses on the direct needs of the individual and their family.
- Support people to improve and maintain their mental, physical, behavioural health and wellbeing, promoting health and preventing ill health.
- Develop understanding of all elements of the nursing process and be able to assist the registered nurse in the ongoing assessment, planning, management and evaluation of care
- To recognise changes in a patients’ condition which require the intervention of others and understand when to refer to the supervising registered nurse and other senior colleagues for reassessment of patient care.
- To work in collaboration with others to ensure the safe and timely discharge of patients with the aim of avoiding readmission.
- To perform procedures as detailed within the NMC Standards for proficiency for Nurse Associate (2018) (Annexe B) to meet the nursing needs of the patients in their care
- To work within the NMC code of professional practice, and within policies, procedures and guidelines of Calderdale & Huddersfield NHS Trust.
- To have a willingness to undertake additional skills and training dependant on department requirements.
- To ensure that high standards of nursing care are given and maintained. To act and raise concerns when standards are not being maintained following local policy and procedures.
- To act as an advocate for the patient and the family, including promoting and protecting their interests, privacy and rights, equality and inclusion.
- To be involved in Trust Initiatives to improve patient safety and care
Person specification
QUALIFICATIONS / TRAINING
Essential criteria
- Nursing Associate Foundation Degree from an approved nursing associate programme.
- Registered Nursing Associate on NMC Register
- GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent
Desirable criteria
- Additional post- registration qualification relevant to field of practice
KNOWLEDGE, EXPERIENCE & EXPERTISE
Essential criteria
- Relevant experience in the field of area applied for
- Relevant experience of working with patients and families from a range of cultures and backgrounds
- Relevant experience of working self-directed or as part of a team
- Understanding the scope of the nursing associate role in the context of the multidisciplinary team and the organisation, and how the role will contribute to service development
- Relevant knowledge related to best practice guidance for care of patients and their families. Ability to communicate with members of the public and health and care providers
- Understands and acts in line with NMC professional standards for practice contained within the Code
- Understand the importance of following procedures and treatment plans
Desirable criteria
- Venepuncture and cannulation/extended roles and skills relevant to work area
- IT awareness and ability to use NHS systems
COMMUNICATION AND RELATIONSHIPS (INCLUDING MANAGEMENT RESPONSIBILITIES)
Essential criteria
- Experience of working in teams under appropriate supervision as part of a multidisciplinary team
- Excellent written and verbal communication skills Ability to communicate effectively (written, verbal, non- verbal communication) with patients/relatives, carers and other members of the multi-disciplinary team
- Ability to organise and prioritise delegated caseload
- Ability to deal with non-routine and unpredictable nature of workload and individual patient contact
- Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
