This is an exciting opportunity to join the Physical Health Monitoring and Improvement Team. We have two opportunies available; one 22.5 hours per week permanent post and one fixed term opportunity, 30 hours per week, covering maternity leave for 12 months - fixed term contracts or secondments considered. During your time with us you will have the opportunity to develop your physical health assessment skills while working alongside a dedicated team of professionals.
This position requires the worker to coordinate and work clinically within daily clinics for:
- the monitoring and management of patients prescribed clozapine in the community.
- administration of depot medications.
- monitoring of the physical health of service users prescribed amber and red medication under the care of LYPFT.
The worker will be required to have the skills and knowledge required to perform venepuncture, ECG and basic physical health observations and escalating and interpreting these observations and providing advise with regards to these results.

Will participate in supervision/ PDP and offer support and training to colleagues as appropriate.
To demonstrate appropriate judgement in unpredictable situations of risk and manage these in a safe, professional, and holistic manner using FACE risk assessment and management plans and understands the need to seek appropriate advice and support
Will be competent in all aspects of clinical communication including the maintenance of accurate records and will show a commitment to further develop these skills.
To offer support and advice to other members of the team concerning clinical practice issues.
To recognise the boundaries of your own knowledge and skills and seek support when appropriate.
Ability to travel to different sites as required.
