Are you a pharmacist looking for a change?  This is an exciting opportunity to join a fairly new team and provide expert support to the service and to patients.

We are looking for an enthusiastic pharmacist who will embrace working within the community setting within Swindon. We are looking for a senior pharmacist (band 8a) but would also consider a development position (band 7-8a) if this is something you are looking towards.

You will be a senior pharmacist supporting Hospital at Home and Urgent Community Response.  Hospital at Home provides a safe and efficient alternative to NHS bedded care providing treatment and care in a patient’s own home. This supports early discharge out of hospital and prevents avoidable admissions. You will provide a highly specialist clinical pharmacy service to virtual ward patients, optimising medicines and ensuring agreed changes in medication are implemented and sustained. This could involve remote consultations, home visits and attending multidisciplinary team meetings. You will be involved in developing and delivering safe and effective pathways of care to patients for a range of conditions. You will also be a key part of a collaborative approach to Hospital at Home across the BaNES, Swindon and Wiltshire (BSW) Integrated Care Board (ICB).

  • Lead, develop and oversee the clinical pharmacy service provided to Hospital at Home and Urgent Community Response in line with national standards
  • Carry out medicine reviews virtually and face to face in patient homes, as necessary
  • Work as an independent prescriber (when trained), optimising medicines and promoting appropriate evidence-based and cost-effective prescribing
  • Utilise clinical assessment skills, where competent and appropriate, to support clinical review of patients within Hospital at Home
  • Provide professional advice and support during multidisciplinary ward rounds
  • Assist in the management of prescribing across the primary/secondary care interface to promote a unified and coordinated approach
  • Provide advice and support to patients / carers and health & social care professionals on medication including education around usage and storage
  • Support and develop the admission and discharge process to and from Hospital at Home and Urgent Community Response services to ensure safe transfer of care in relation to medicines
  • Co-ordinate the day-to-day activities of the pharmacy service, supervising and line managing pharmacy technicians and rotational pharmacists within the service
