Job overview

This is an opportunity to work within the Swindon Community Hospital at Home- Virtual Ward service which is currently developing and expanding. The service provides an innovative way of offering patient care in the community and optimising early discharge. A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology. Virtual wards support patients who would otherwise be in hospital to receive the acute care, monitoring and support they need in the place they call home. This includes either preventing avoidable admissions into hospital or supporting early discharge out of hospital.

We are looking for Consultant/s or General Practitioners who would be interested in covering up to six sessions per week within the Hospital at Home - virtual ward.

NHS England and NHS Improvement are now asking all integrated care systems (ICSs) to extend or introduce for the first time the virtual ward model with an ambition for all systems to deliver virtual ward capacity equivalent to 40 - 50 virtual ward beds per 100,000 population (equivalent to the delivery of up to 24,000 virtual ward beds).

The Virtual Ward will provide senior clinician led care for a variety of patients including those with respiratory conditions and Frailty. This care is provided to them in the comfort of their usual place of residence using advanced personal remote monitoring equipment, enabling them to safely leave hospital sooner or not come into hospital at all.

 

 

Main duties of the job

The medical provision within the virtual ward is an integral part of providing a multi-disciplinary approach to person centred care. The postholder will be delivering care, provide treatment and intervention to meet individuals needs within the community and in the Community services Hub based at the Orbital.

The service has multi-professional input from consultants, specialist nurses, advanced care practitioners and a wealth of other health and social care staff. The team provides holistic person-centred care with the aim of preventing unnecessary hospital admission or supporting early discharge in a variety of ways.

Virtual wards provide acute clinical care at home for a short duration (up to 14 days) as an alternative to care in hospital. Patients have their care reviewed daily by the senior clinicians and virtual ward team. Some patients will be monitored via a digital platform that allows for the remote monitoring of a patient's condition and escalation to a multidisciplinary team. There will be requirement to review and prescribe medications for patients, to help them remain at home.

Our  clinicians play a pivotal role on-boarding and discharging virtual ward patients ensuring equipment is used safely, following relevant training, and that it is maintained appropriately. The post holder will complete delegated assessments and apply competency-based treatment, care and interventions to patients.

Qualifications

Essential criteria
  • Fully registered with the GMC with license to practice
  • MRCP or Equivalent

Clinical Experience

Essential criteria
  • Clinical training/experience equivalent to that required for gaining UK CCT
  • Broad range of recent experience within general internal medicine
  • Ability to lead a multi-professional team and take full and independent responsibility for clinical care of patients
  • Evidence of ability to change clinical practice and service provision
  • Up to date practice in line with recent initiatives

Teaching and Research

Essential criteria
  • Ability to teach clinical skills to trainees and multi-disciplinary teams
  • Interest in leading multi-professional education
  • Publications in peer reviewed journals
  • Ability to supervise post-graduate research
