Full and Part Time Positions Available
An exciting opportunity has arisen for one full time and one part time position as a Cancer Patient Navigator in a progressive Cancer Services Team.
We are looking for a motivated, organised and enthusiastic team player to join an established team. Training will be provided to support a new starter in this role. The individual will have an eye for detail ensuring accurate data input in line with National Cancer Waiting Times Guidance, Quality Surveillance and COSD and good communication skills
Main duties of the job
This important role is to help support and navigate patients, their families and carers through Cancer pathways ensuring that the service’s prime aim is to be patient-focused and proactive in seeing that the patient receives the very best cancer pathway experience within the shortest time frame with continual communication and support.
The role requires someone with exceptional organisational skills, who can demonstrate great communication and interpersonal skills. A caring and sensitive attitude is essential as you will be dealing with patients, their families and carers. The individual will be expected to work independently within the appropriate policies and procedures for the service.

Detailed job description and main responsibilities
- Track & chase diagnostic investigations for all patients on specific cancer pathways ensuring these are booked to meet timed pathway requirements. Ensure results are available for Outpatient appointments and MDTs.
• Proactively liaise with patients on cancer pathway to ensure they are aware of diagnostic pathway
expediting investigations whilst meeting their preferences reducing risk of wasted appointments.
• Liaise with GP practices to retract referrals where patients are not engaging in the cancer pathway or inappropriately referred.
• Book post-MDT follow up appointments for certain tumour sites
• Track & chase surgical/tertiary hospital investigations including PET scans
• Attend cancer PTL (Patient Tracking List) meetings for relevant tumour sites.
• To carry out administrative and clerical duties as requested within the department in an efficient and confidential manner.
• To assist the team in collating excellent website- cancer related information.
• To communicate with patients families and carers in a sensitive and confidential manner working closely with clinical teams as required.
• Use electronic hospital systems to pull of results or information as required. - The post holder will ensure records are maintained on hospital IT systems.
• The post holder will be expected to exercise judgement when dealing with enquiries and analyse information to resolve the patient queries.
• The post holder will support the team in managing diaries, arranging meetings, scheduling clinics.
There is an expectation to plan and organise straight forward activities on an ongoing basis.
• Minute taking will be required at times.
• The post holder will support audits and survey completion within the department gathering data as required.

Qualifications
Essential criteria
- Educated to at least A-Level/diploma level or equivalent
- Demonstrable evidence of recognised IM&T qualifications (minimum equating to ECDL)
- Demonstrable evidence of further (relevant) Continual Professional Development
Desirable criteria
- Keyboard skills to RSAII or equivalen
- Database management
- Oncology for non-clinicians and/or National Cancer Intelligence Network elearning cancer modules
Experience
Essential criteria
- An understanding of medical terminology relevant to the role
- Able to demonstrate experience of effective working in a team environment (any setting)
- Previous experience of coordination of a service
- Previous experience of using data validation and extraction software
Desirable criteria
- Familiarity with patient clinical notes and data systems
Knowledge
Essential criteria
- Able to demonstrate high level of accuracy
- Able to demonstrate analytical skills/reasoning
- Able to demonstrate facilitation and influencing skills
- Able to demonstrate organisation and administrative skills
Desirable criteria
- An understanding of national programmes relating to Cancer Care and cancer patient pathways
