Job overview

An exciting opportunity has arisen for a General Practitioner with an interest in medicine for older persons to join a fantastic team within our Integrated Care and Community Division, as a GP on Orchard one of our Intermediate Care Wards. This post is a great opportunity for a clinician with vision and passion for Intermediate Care to join us.

Relocation expenses of up to £8,000 are available to the appointed candidate (following the policy).

Applications are invited from General Practitioners with experience in Geriatric Medicine, with a broad base of training in General Practice. You must hold full registration with the General Medical Council and be on the GMC GP Register.

Main duties of the job

This part time role is a replacement post within the Orchard Ward medical team, the appointed candidate will join the existing GP in Intermediate Care in a job share covering at least half the week Wednesday to Friday with hours to be negotiated. You will help us continue to drive forward improvements in the clinical services that we provide to our patients, and as a senior member of the team, you will become a key part in the supervision and delivery of patient care within Orchard Ward our 30-bed step down ward.

You will be passionate and enthusiastic about older persons services and the development of integrated pathways that support great patient care. You will have the focus and skillset to help develop patient centred pathways that create capacity and help to develop flexible and transformational ways of working alongside partner agencies and the Integrated Care Alliance.

You will deliver an excellent standard of clinical care and co-lead the clinical team in a busy inpatient area adhering to the GMC standards for good medical practice and contributing to the effective management of the unit. You will lead by example and maintain positive collaborative working relationship with the multi-disciplinary team and partner agencies.

Intermediate Care is part of the Integrated Care and Community Division which is one of three Trust divisions and is a large and diverse Division consisting of Community Nursing, Virtual Ward, Therapy services, Urgent Care response, End of Life services, Outpatients, Cancer services and community beds. 

Orchard Ward is located in the SWICC building on the main hospital site, which has excellent travel links (including good onsite parking), with many cities within easy range. The Trust is located on the edge of Swindon with access to a variety of outstanding areas to live which include both urban and village areas with short commutes to the site.

Detailed job description and main responsibilities

The post holder will deliver an excellent standard of clinical care and co-lead the clinical team in a busy inpatient area adhering to the GMC standards for good medical practice and contribute to the effective management of the unit, leading by example and maintaining a positive collaborative working relationship with the multi-disciplinary team.

The post holder will be comfortable working within a high-pressured environment with enthusiasm whilst providing safe care. Working closely with the rest of the MDT to help with innovative discharge pathways, challenge blocks and encourage appropriate risk taking with discharges. You will join our existing GP working on Orchard Ward to provide joined up care and clinical leadership with a strong emphasis on great care. You will be part of a dedicated medical team including Junior Doctor and ACP workforce, and working alongside the ward manager, skilled nursing, specialist therapy and dedicated discharge co-ordinators to provide seamless care and early supported discharge with a focus on home wherever possible.

Qualifications

Essential criteria
  • GMC Registration with a licence to practice
  • Entry on Specialist Register for Geriatric Medicine / General Medicine or completed GP training or fully qualified GP with GMC
  • Membership of Royal College of General Practitioners – MRCGP

Experience

Essential criteria
  • Ability to lead a multi-professional team and take full and independent responsibility for clinical care of patients
  • Experience of inpatient rehabilitation

Knowledge

Essential criteria
  • Expertise in full range of Elderly Care conditions
  • Clinical skills required to manage a wide range of patient care needs
  • Sound knowledge of clinical governance, professional accountability and responsibility
