Six Months - Fixed Term - Maternity Cover.

This is an exciting opportunity for a dynamic and highly motivated nurse to join the Macmillan Personalised Care Team at The Great Western Hospital to help support the NHS Long Term Plan 2019 that "every person diagnosed with cancer will have access to personalised care, including a needs assessment, care plan and health and wellbeing information and support." 

(This also presents a secondment opportunity)

Prif ddyletswyddau'r swydd

The post holder will be collaborate with the MacMillan Personalised Care CNS Team Leader to specialise in the field of cancer care, influencing, developing and expanding cancer services in delivering Personalised Care to people with cancer throughout the Trust. The team also has a Macmaillan physio and OT who the post holder will be working closely with. 

The post holder will work closely with health care professionals from primary and secondary care settings to ensure that the delivery of the personalised care agenda within the Trust is consistent with both national and local strategy, to include relevant governance, quality and operational frameworks.

The MacMillan Personalised Care CNS is responsible for the development of a high quality, effective and efficient service to patients and healthcare professionals through leadership and supervision, robust clinical governance and education.

1. To work with the Macmillan Personalised Care CNS Team Leader to deliver Personalised Care for People Living with Cancer.

2.To be involved in the future development and expansion of a high quality, effective and efficient Macmillan Personalised Care Team.

3. To work alongside Oncology Clinical Nurse Specialists and Patient Pathway Facilitators to deliver high quality Personalised Care.

4.To deliver both face to face and online workshops for both patients and staff to support the Personalised Care Package.

5.To deliver and lead in the provision of Holistic Needs Assessments within Cancer Nursing.

Experience

  • • Health and Safety in relation to the specialty.
