Job overview

Herefordshire & Worcestershire ICS is committed to using strategic and outcomes based commissioning to transform service delivery built around the patient and neighbourhoods. Our approach to implementing the Fuller Stocktake locally is set out in the ICS Delivering Good Health & Well Being for Everyone Programme. This spans 3 years, encompassing co design and production with our two Places, population health management and new ways of commissioning and incentivising providers.

We are a new directorate within the ICB and ICS and cover interconnected portfolios such as Frailty, Care Homes, development of Integrated Neighbourhood Teams, Same Day Urgent Access, local GP commissioning, and this portfolio is expected to increase in 25\26. Main duties of the job

The post holder will contribute towards the implementation of Delivering Good Health & Wellbeing programme. Lead areas will be agreed upon appointment.  This includes supporting the redevelopment of commissioned services via outcome-based commissioning, to support the development of Integrated Neighbourhoods Teams, and improve access to health and care, and outcomes for residents.

We are looking for a motivated self-starter who is highly organised and passionate about improvement in health care, bringing with them a strong track record and experience of leading transformation, integration and improvement projects. 

Experience of working in an agile and demanding environment, providing expert leadership in interpreting, communicating and implementing strategic priorities is key.

Detailed job description and main responsibilities

The post holder will support the development of effective and efficient integrated Primary & community services with a specific focus on the delivery of the 3 year Delivering Good Health & Well Being Programme.  The key elements of the role are:

Lead & support the delivery of programmes, projects and workstreams associated with the Delivering Good Health and Wellbeing programme and associated priorities across the system, Place & Neighbourhoods. With a focus on the redesign of integrating services in priority areas, improving outcomes for patients and delivering improved productivity efficiencies.

Lead and support programmes and projects across the Directorate’s portfolio. These include but are not limited to, Integrated commissioning of services, transformation, delivery of the frailty strategy, Care Homes, Primary Care and workforce development and transformation.

Be the strategic commissioning & development lead for services which sit within the Directorate’ s portfolio: Interpret and apply legislation, guidance, local and national priorities to inform the development of plans and strategies and advise colleagues, partners and senior managers; make informed commissioning decisions that are good value for money, address local need and outcomes and deliver local priorities.

Provide support to the emerging Integrated Neighbourhood Accelerator Sites. 

Delivery of required system wide best use of Resources Programme outputs; identify and develop plans to support system improvements that also reduces system cost where possible. Monitoring and evaluating projects, demonstrating benefits and impact.

To work collaboratively with partners and residents to develop outcome focused strategies and service specifications with clear priorities and targets for achievement that support strategic development.

Development and delivery of improvement action plans in relation to agreed priorities and commitments.

Identify areas of need or gaps in provision including health inequalities in our communities.

Develop effective relationships with the local authorities, provider organisations and member GP practices that support the delivery of effective health and care services.

Undertake engagement events and opportunities with patient representatives, patient groups and other stakeholders to ensure that lived experience and front-line practitioners’ experience inform the scope, delivery and decision-making of projects.

Lead as required to the coordination of training, development and recruitment activity across the Team.

Qualifications

Essential criteria
  • Relevant degree-level qualification or equivalent professional qualification or management experience or knowledge acquired through experience or training
Desirable criteria
  • Post-graduate qualification or equivalent experience in healthcare commissioning
  • QSIR / PRINCE 2 or similar project management qualification

Experience

Essential criteria
  • Demonstrable experience in a similar post, working in a transformational or improvement role.
  • Leading successful service re-design projects in relation to healthcare
  • Demonstratable positive leadership and management behaviours, and matrix working experience
Desirable criteria
  • Demonstrable experience in the redesign or development of pathways across Primary care, Community, or acute hospital services, including experience of quality improvement methodology
  • Demonstrable experience of system leadership

Skills, Knowledge & Competencies

Essential criteria
  • Ability to adopt systems leadership behaviour and implement these values across a complex health and care systems to influence and effect system change
  • Up to date professional and technical expertise in relation to planning, commissioning, procurement, evaluation, performance management and reporting. With the ability to relate this to designated areas
  • Ability to communicate effectively and accurately at all levels, including ability to write reports, business cases, briefings, and present information clearly in meetings and events

  • Understanding of population health management
  • Knowledge and understanding of the local Herefordshire and Worcestershire Health and care system, and local population
