We are looking for a Clinical Pharmacist to join our innovative team. 

The NHS@Home service - our Virtual Ward - offers an alternative to hospital care. It combines innovation and technology with face-to-face treatments for people who are acutely unwell in their own homes across BNSSG. 

The service is run in partnership between Sirona, North Bristol Trust (NBT), University Hospitals Bristol and Weston NHS Foundation Trust (UHBW) and BrisDoc. 

We are looking for a patient-focussed and forward-thinking clinical pharmacist for this interesting and varied role. The successful candidate will truly work across the interface between primary, community and secondary care, spending time providing pharmaceutical care for NHS@Home patients onsite at Weston General Hospital, as well as in patients’ homes across Weston-super-Mare and neighbouring areas.

There is a strong element of cross-organisational working to the role, requiring close liaison with the rest of the NHS@Home team, as well as the clinical & pharmacy teams within UHBW. The NHS@Home pharmacy team is friendly and supportive, with pharmacists and pharmacy technicians in both of our Acute Trusts and Sirona.

The successful candidate will be able to use their clinical skills across the range of NHS@Home clinical pathways.

Main duties of the job

You will work alongside NHS@Home pharmacy colleagues based at UHBW, NBT and Sirona, to deliver and develop the clinical pharmacy aspects of the NHS@Home service across BNSSG.

You will contribute to the clinical pharmacy service for NHS@Home patients. Currently the following 
clinical pathways are delivered:
•    Heart Failure
•    Respiratory
•    Frailty
•    Outpatient Parenteral Anti-microbial Therapy (OPAT)
•    General pathway (for a range of general medical & surgical conditions)

You will contribute pharmacy skills and expertise to the NHS@Home multi-disciplinary team, which consists of pharmacists, pharmacy technicians, Advanced Clinical Practitioners, nurses, nurse associates, doctors, paramedics, physiotherapists, occupational therapists, support workers and other allied health and care professionals.

This post will be primarily based in North Somerset and will require the successful candidate 
to:
•    Work closely with the NHS@Home and Pharmacy teams at UHBW.
•    Work in an agile way to deliver pharmacy services to NHS@Home patients:

     - At Weston General Hospital

     - In their home/other place of residence across Weston-super-Mare (WSM) and neighbouring areas.

Travel will be required across the BNSSG area for meetings and other duties.

You will need to be a driver with access to your own vehicle and will be eligible to claim travel expenses.

Aside from delivering clinical services, the post involves a range of management & leadership roles.

Qualifications

Essential criteria
  • Pharmacy degree (Mpharm or equivalent) plus 1 year pre-registration/foundation training
  • Practising Pharmacist registered with GPhC
Desirable criteria
  • Post-graduate qualification in clinical pharmacy, therapeutics or pharmacy practice, or equivalent experience.
  • Non-medical prescribing qualification

Experience

Essential criteria
  • Post-registration experience in a clinical setting/relevant role
  • Experience of working with a multidisciplinary team.
  • Experience of undertaking medication reviews with patients.
  • Experience of providing prescribing and medicines optimisation advice to health professionals.
  • Experience of implementing new procedures or guidelines.
  • Experience of audit techniques.
  • Evidence of continued professional development (CPD).
Desirable criteria
  • Experience of line management or supervision of individuals.
  • Experience in the delivery of education and training.
  • Experience of contributing to policy or procedure development.
  • Clinical knowledge and experience of antimicrobial therapies and antimicrobial stewardship.
  • UK hospital pharmacy experience
  • Experience of working with a Virtual Ward service
