Job overview

An exciting opportunity has arisen for a suitably qualified and enthusiastic nurse to join this well established cardiac rehabilitation service. The post holder will join the multidisciplinary team to contribute to the delivery of cardiac rehabilitation care in a variety of settings. The role will involve leading on and implementing cardiac rehabilitation interventions in Wrexham with requirements to travel across Wrexham and Flintshire.

The client caseload includes those following acute coronary events and /or coronary intervention and those with heart failure. The postholder will promote independence, confidence and improved quality of life for both the client and their family where necessary. You will require excellent communication and motivational skills and be able to work effectively both within a team and on your own.

This post will be suitable for someone with relevant experience in cardiac nursing and/or nursing individuals with long-term conditions in primary or secondary care. It may attract nurses from a variety of settings eg. coronary care, emergency department, practice nursing, district nursing.

Main duties of the job

Daily delivery of cardiac rehabilitation nursing interventions within the context of the cardiac rehabilitation multidisciplinary team, communicating with acute and community and third sector services and practitioners, to promote effective prevention and rehabilitation for the patient.

Identify and provide timely triage of appropriate patient referrals. This will involve the interpretation of cardiovascular test results in relation to the patients’ presentation and other health needs.

Provide specialist, holistic patient assessment to include cardiovascular lifestyle risk factors, medical history, psychosocial health, medical risk factor management. The nurse will be required to use highly skilled communication to elucidate important, often sensitive, information and needs that impact upon the patients’ cardiac event, recovery and long-term management.

 Possess specialist knowledge of and use a variety of generic and disease-specific assessment tools based on the presenting needs of the patient, interpreting the results to inform patient care.

Request, organise and interpret relevant clinical investigations as required eg. exercise treadmill tests, blood profiles and ECGs. Review with or initiate direct referrals to other health care professionals i.e. cardiologists, physicians and GPs to inform the development of treatment and management plans.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications

Essential criteria
  • NMC Registered Nurse
  • Educated to degree level or equivalent level of experience
  • Post-graduate level module in cardiac care or community care or in nursing patients with long-term conditions
  • ILS trained or agreement to undertake
  • Mentorship course or agreement to undertake
  • Agreement to undertake relevant cardiovascular care training
  • Familiarity with the practical application of the core components of cardiac rehabilitation ie BACPR standards and core components for cardiovascular prevention and rehabilitation.
  • Agreement to undertake further training in motivational interviewing and/or counselling and/or advanced communication
Desirable criteria
  • Relevant course or training in cardiovascular care
  • Training in motivational interviewing and/or counselling and/or advanced communication
  • Understanding of National Audit of Cardiac Rehabilitation
  • Knowledge of relevant National guidelines eg NICE, ESC, Welsh Government
  • Non-medical supplementary / independent prescriber
  • Advanced clinical examination skills

Experience

Essential criteria
  • Significant post registration experience
  • Post-registration experience in cardiac or community nursing or managing patients with long-term conditions
  • Experience in teaching / facilitating learning eg through nursing role or other role
Desirable criteria
  • Experience of coordinating and managing patient care
  • Experience in managing patient groups or groups of people
  • Has undertaken audit activities
