Job overview
A vacancy has become available for a permanent Senior HCSW within Evington Ward, Rehabilitation Unit for the Elderly at the WMH.
Our aim is to provide quality, planned care for the patients. The successful candidates will work and support the Discharge Facilitator. Support the team in the management of patient’s medical case notes and effective filing system and supporting the patients along the defined patient pathway
The post holder will promote and build positive relationships within the Discharge Team, patients and service users and all BCU employees. Contribute to the effectiveness of the medical secretary team.
The post holder will undertake a broad range of duties and will be expected to work independently and as part of the team
The post holder will be able to deliver elements of health and social care by having the knowledge and assessed level of competence to undertake such a role which will be locally assessed
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
As requested by the Discharge Facilitator carry out duties and tasks that are relevant to the role, ensuring that productive working relationships with colleagues are maintained and developed.
Plan and coordinate appropriate resources in preparation for specific activity or tasks as requested by the Discharge Facilitator or management team, liaising with multi disciplinary staff to ensure the smooth, efficient and professional delivery of the activity
Be proactive and use initiative to ensure that work tasks adhere to the agreed operating instructions and Key Performance Indicators for the speciality as set by the line manager.
The post holder will provide support in the training of new starters promoting and building positive relationships within the Discharge team, patients and service users and all BCU employees.

Qualifications
Essential criteria
- QCF Level 3 or equivalent qualifications/experience NVQ level 3 in Health & Social care related subjects or equivalent qualifications/experience Good knowledge of anatomy & physiology
Desirable criteria
- All Wales HCSW induction framework
Experience
Essential criteria
- Experience of working as a HCSW without direct supervision in a clinical setting and being managed by a qualified practitioner. Good understanding of risk assessment and management
Desirable criteria
- Any other specific clinical skills at this level of practice pertaining to role as per service need
Skills
Essential criteria
- Evidence of Being able to provide wound care using aseptic technique ANNT Phlebotomy Basic observations The ability to communicate clearly and accurately. The ability to be adaptable to new and changing circumstances.
Desirable criteria
- The ability to work alone under supervision and to understand when to seek clinical advice and support from the qualified Nurses and Knowledge and understanding of clinic activities
