Care Coordination Centre Team Manager

Job overview
The Care Coordination Centre provides high quality, effective and timely administration service to internal and external customers, ensuring that stakeholder’s information requirements are fully understood and that the service provided meets those needs. The operational manager will ensure that the work of the team is undertaken in a timely and efficient manner whilst complying with the relevant policies, procedures and protocols including those relating to patient confidentiality, information sharing, records management, data protection, Caldecott principles and Information Governance.
Main duties of the job
· Have a high-level customer focused approach for the service delivery and always ensuring this is maintained within the admin team.
· Line management of Team Leaders within the service.
· Undertaking performance and development reviews and supporting others to create personal development plans related to their job roles and aspirations.
· Support the Team Leads in managing sickness absence with the support of the HR department and consider ways to minimise work absence.
· Investigate and action reported incidents with use of Datix.
· Collaborating across the service in developing staff rotas and making best use of available resources.
· Specialist knowledge of a clinical service area or range of services supported by the Care Navigators.
· Responsible for complex data collection about the services managed by the single point of access as required.
· Systems Super User – available for advice and support to administration and clinical/medical staff.
· Create and maintain comprehensive service documentation, plans and reports.

Detailed job description and main responsibilities

· Answering queries, progress chasing, task-related problem solving and resolving issues for clinical/medical staff or patients and providing non-clinical advice or information to others.
· Data inputting into clinical and administrative systems.
· Liaise with and support operational leads/managers and staff members around service issues.
· Provide advice and potential solutions to mitigate service risk.
·Responsible for the implementation of standard processes/procedures and contributes to service development through making suggestions at team meetings etc.
· Developing and updating induction processes for new starters.
·To contribute to achieving the KPI’s, service level agreements and implementing benchmarking/standards.
· Assist with continuous service improvement, including proposing and supporting changes to processes and tasks across a range of services.
· Establishing effective relationships with external contacts and partnership agencies and ensuring Provide is represented as an outstanding organisation.
· Providing cover and working co-operatively with others.
· Will be responsible for signing off travel expenses.
· Undertake work related to direct line reports.
· To work on any other projects or other work in the department as requested by the manager.
· To be visible to support staff and operational leads/managers as required.
· Managing the budget for CCC if required.
· Any other tasks as required that are commensurate with the role.

- Degree level or equivalent work-related experience

- Evidence of commitment to CPD Qualification in a service area specialism

- An understanding and knowledge of the main health issues affecting the local population
Applicant requirements
The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
