Job overview
The Hospital at Home service is expanding and has vacancies for the newly created Health and Care Assistant roles. The successful candidates will work alongside the Discharge Home to Assess team to predominately provide short term Care to prevent patients requiring hospital admission.
The roles are based within the East and North corridors and staff will be expected to travel within the corridors,
Previous care experience is essential.
The service runs 7 days a week, 8am to 8pm.
Main duties of the job
- Providing personal care to patients within their own home
- Following training to provide basic health care needs as required

Detailed job description and main responsibilities
- To work within a team that provides an assessment and care service to patients with nursing / health needs.
- To visit patients in their own homes and provide appropriate care and support as defined in the Care Plan. This will include aspects of personal care, hygiene, uncomplicated nursing procedures, exercise, activity and treatment programmes prescribed by therapists. The aim is to rehabilitate and maximise the patient’s potential and where appropriate to support the carer.
- To support and enable patients in a range of activities to promote independent living and monitor their well-being.
- To undertake administration and clerical tasks that are associated with the care process including record keeping and use of computer.
Word versions of documents are available if needed to allow for any adaptations to be made i.e. font, colour, background, convert to Read Aloud.

Qualifications & Training
Essential criteria
- Good general education
- NVQ Level III, or equivalent knowledge and experience.
- Basic mathematical skills

- Excellent written and verbal English skills
