Are you interested in nutrition, and passionate about improving health and patient care? An opportunity has arisen to work as a Dietetic Assistant Practitioner (location flexible) to support nutrition screening of inpatients to limit frailty and improve health outcomes. You will be well supported by our enthusiastic and friendly community clinical team Dietetic services across Powys.
This is a fixed term opportunity during which we will aim to gather data to support ongoing funding by demonstrating impact and support to busy ward staff, patients and families. Time will be spent visiting PTHB wards (face to face where practicable), or by TEAMS promoting nutrition screening and appropriate actions such as provision of supplements, fortified foods and liaison with wider heath teams. Strong communication, IT and teamworking skills are essential, well suited to a HCSW who knows PTHB, is interested in nutrition, and who seeks personal development.
Main duties of the job
Delivers high quality, compassionate, evidence-based care to a group of patients following the initial assessment by the Registered Practitioner utilising person-centred approaches which promote health and independence.
Under leadership and direction of the Registered Practitioner; ensure that all patients have appropriate individualized, clinical assessments/ risk assessments providing accurate feedback to the team as necessary.
Effectively monitors the patient’s condition recognising deterioration in their physical or mental condition and liaises with the Registered Practitioner in an appropriate and timely manner to review / reassess care needs as required.
Contributes to the on-going assessment of the patient providing information to inform assessments and discussing any changes to planned care with the Registered Practitioner.
Demonstrates effective approaches to monitoring signs and symptoms of physical, mental, cognitive, behavioural and emotional distress, deterioration or improvement.

Detailed job description and main responsibilities
As an Assistant Practitioner the post holder is expected to independently manage their own work and case load, undertaking tasks delegated by a Registered Practitioner with appropriate supervision in place. Having an understanding of evidence-based practice and delivering care in line with current evidence, take responsibility for taking action relative to an individual’s health and care in accordance with organisational policy and procedures.
Responsible for some elements of assessment, implementing programmes of care and modifying individualised care plans, reporting back to the Registered Practitioner. They may delegate work to others and may supervise, teach and assess other staff.
The ability to speak Welsh is desirable for this post, but English and/ or Welsh speakers are equally welcome to apply.

Essential criteria
- Certificate in Healthcare (120 Level 4 CQFW Credits) or NVQ Level 3/ A-Levels/ BTEC in Healthcare associated certificate with significant demonstrable experience in the field of specialism
- Demonstrates an understanding of the role of Assistant Practitioner in the context of the weight management team and interdisciplinary team and the organisation
- Demonstrates understanding of the importance of following procedures and treatment plans
- Demonstrates knowledge of when to seek advice and escalate to the appropriate registered professional for expert help and advic
- Demonstrates an understanding of the legislation related to Safeguarding
- Good understanding of person-centred care
