Come and join our innovative team in Canolfan Goffa Ffestiniog Health Centre. We are looking for an Advanced Clinical Practitioner to join the team providing patients with the opportunity to see the most appropriate
practitioner to effectively deal with their presenting condition at the first point of contact.

Main duties of the job

This is an exciting opportunity to join the friendly GP practice team as an Advanced Clinical Practitioner for the Health Board Practice at Canolfan
Goffa Ffestiniog, Blaenau Ffestiniog .  We offer a Multi-disciplinary Team model, providing patients with the opportunity to see the most appropriate practitioner to effectively deal with their presenting condition at the first point of contact.
The extended GP practice team consists of Salaried GPs, Practice Nurses, Health Care Assistants, Medicines Management Team, Advanced
Physiotherapist and Occupational Therapist. The environment is of a happy and supportive nature with the team looking after each other and is always
looking at innovative ways of working. Clinical and peer support is available.
We are looking for a person with a proven record/special interest in caring for patients with minor illness and some experience of dealing with
Chronic Disease Management.
Ideally the candidate will hold an MSc in Advanced Clinical Practice or working towards this. 

This post will require the successful applicant to visit patients in their own homes, lead on Care Home visits, Palliative Care, CRTs and other general practice dependant on experience.

The Health Centre is open between 8.00 and 6.30 pm, the post holder will be expected to work during these hours.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

  • BSc/ Grad Dip in relevant discipline
  • Masters level education or equivalent experience; e.g. Advanced clinical practice or working towards a Masters
  • Experience of Primary Care
  • Active professional registration
  • Evidence of CPD and post registration activity through portfolio
  • Extended scope skills
  • Independent prescriber or Working Towards

  • Experience of acute care i.e. OOH
