As a Triage Clinician, you will be given a high degree of autonomy to reach a safe and appropriate disposition that may include providing advice and guidance, onward referral to a Sirona team or signposting to the most appropriate service for their needs, always being an advocate for the patient, SPA, own profession and for Sirona.

You will work alongside our diverse team of Triage Clinicians and health navigators who are responsible for processing the routine referrals and urgent callout requests that are received for adult patients within Sirona boundaries.

You will work alongside a compassionate leadership team who accommodate their staff. We can offer full time, part time and flexible working patterns. Hybrid working is available after your initial 6-week training.

As a SPA Triage Clinician, you will be working across the following areas:

  • Warmley Busines Park - South Gloucestershire
  • Withywood Centre - South Bristol 
  • Castlewood - Woodspring

This is a fast paced and dynamic environment operating between the hours of 07:30 and 20:00.

Main duties of the job

In this role, you’ll be working as a SPA Triage Clinician, ensuring everyone who accesses the service receives a high quality of care that is underpinned by the Sirona values that revolve around “taking it personally.”

Utilising your clinical knowledge, problem solving ability, leadership skills and experience, your responsibilities will include:

  • Person-centred triage of referrals using telephone and/or video consultation methods in the future.
  • Providing advice and reassurance to patients, their relatives, friends and carers.
  • Raising any safeguarding or other concerns identified in compliance with the organisations policies and procedures.
  • Identifying service users’ needs and establishing the timeframe in which care is required including those needed to help in preventing unnecessary hospital admissions or to facilitate an early discharge from hospital.
  • Allocating triaged referrals to the correct service and signposting to other partners as appropriate.
  • Supporting clinical and non-clinical colleagues within and outside of SPA through shared decision making and professional guidance.
  • Keeping own knowledge and skills current through a commitment to Continuous Professional Development and the timely completion of mandatory and other essential training.
  • Actively participating with individual and group supervision including one to ones and personal development reviews (PDR’s).
  • Maintain complete and accurate documentation using EMIS.

Qualifications

Essential criteria
  • Registered healthcare professional with current registration with a professional body (NMC, HCPC etc).
  • Evidence of recent continuing professional development.
Desirable criteria
  • Post-graduate training in patient assessment and/or hear and treat.

Skills & Experience

Essential criteria
  • Experience within an acute or community setting.
  • The ability to interpret clinical information, using analytical and problem solving skills to inform decision making.
  • Experience of working across organisational boundaries within health, social care and mental health.
  • Experience of recognising and managing the deteriorating patient including an understanding of SEPSIS and NEWS2.
  • Ability to communicate clearly and succinctly in writing with a competent level of IT skills
  • Demonstrable knowledge of key clinical interventions including skin/wound care, continence, and catheter care. Medication administration, IV access and antibiotics, vaccinations and injections, mobility assessments.
Desirable criteria
  • Previous experience of telephone or remote triage and consultation.
  • Understanding of CQC compliance and an ability to work towards required standards
  • Ability to travel between bases
