Patient Flow Coordinator - Part-time, 15 Hours Per Week

Would you like to do a job that makes a difference every day?  An exciting opportunity has arisen for a motivated individual to join the Community Podiatry Team as a Patient Flow Co-ordinator based at City of Coventry Health Centre. 

This is a new role designed to be dynamic and exciting. You will be responsible for managing service waiting lists and identifying fluctuations and rationale for this. You will support patient movement across the service.

You will keep IT systems updated with regard to patient data. You will prepare reports and data with a focus on length of stay. You will support and lead on projects relating to patient flow looking in detail at capacity and demand models. 

You will be a resilient team worker who thrives on the opportunity for a new challenge and to be part of a service delivering high quality care.  You will be a confident communicator as the role requires delivery a range of communication methods to all stakeholders internal and external.

Main duties of the job

  1. To deliver training in Pathways, and system support tools to support assessments as required ensuring safe discharge planning and patient flow coordination with the correct identified level of support
  2.  To support patient flow across the servi ce, ensuring patients have the correct future pathway.
  3. To coordinate the assessments and referrals on patients related to patient flow.
  4.  To work independently, seeking senior level of support as required on tasks outside of identified competency.
  5. Provide education, support and advice to the Podiatry Team on the patient flow and discharge process and pathways.
  6.  To represent Complex Flow Improvement, Lead where applicable and to provide team development and case load updates to Operational Lead/Clinical Lead of any blockages.

Detailed job description and main responsibilities

  • To Support Operational Lead in development and delivery of service strategy and organisational development.

  • To enable capacity to be maximised across the clinical team.

  • Support the clinical team to enable effective patient flow, promoting value added length of stay within the service.

  • Support the Operational Lead in ensuring the improvement and delivery of targets related to Services, and administrative activity targets to meet the demands of service targets, local and national audits that may be required. 

  • Support the Operational Lead in ensuring clear communication, business objectives and agreed targets within the service.

·    Ensure staffing levels meet the service demands on a daily and trend basis. Identifying the need for bank/agency staff and liaise with staff/agencies concerned, in conjunction with the Operational Lead

  • To support services in the improvement and delivery of targets by ensuring effective and efficient usage of capacity within the teams

  • Responsible for the extraction, processing and analysis of data from multiple Trust information systems developing   protocols to support the analytical process.

  • Actively contribute to the progressive development of data quality techniques and information analysis

·       Be aware of service objectives that need to be delivered and targets that need to be met

·      To support the development of the team to work and improve services.

·       To attend and participate in regular team meetings and contribute towards safety and quality meeting agendas.

·       To make competent and conscientious decisions using negotiating and motivating communication skills via telephone or face to face with clinical staff. 

·       Contribute to meaningful data collection to reflect the team’s activity and productivity

·       Participate in maintaining optimal standards of care and service delivery in line with CQC.  Ensure practice of self is based on best available evidence and standards.

·       Undertake Risk Assessments for the department

·       To provide non-clinical interventions such as advice, guidance and direct support to clients on their caseload.

·       To inspire others and encourage them to seek advice and solutions to problems.
